摘　　要：程少丹认为难治性膝骨关节炎核心病机在于肾阳不足,推动无力,导致水液内停,宗筋瘛疭为患。治疗关键在于温阳利水,形成针药结合的温阳利水治法。针具继承上海伤科八大家之陆氏伤科银质针及芒针,针法强调辨证取穴、粗针重刺、长针深刺,温针灸助阳,泻机关之水;遣方用药主以调身通痹方及温肾通痹方随证加减。Cheng Shaodan believes that the core pathogenesis of refractory knee osteoarthritis lies in the deficiency of kid-ney Yang,resulting in insufficient driving force,which leads to the retention of body fluids and affliction of the sinew chan-nels with spasmodic pain.The key to treatment is to warm Yang and promote diuresis,forming an integrated approach of acupuncture and pharmacotherapy to warm Yang and promote water metabolism.The acupuncture implements inherit the silver needles and elongated needles of the Lu-style orthopedics from the eight great Shanghai orthopedic schools.The acu-puncture techniques emphasize syndrome differentiation in selecting acupoints,forceful pricking with thick needles,deep insertion with long needles,and warming moxibustion to support Yang,as well as draining pathogenic fluid from the joints.The prescribed medication primarily consists of bodyregulating and obstruction-clearing formulas,as well as kidney-warming and obstruction-clearing formulas,with modifications according to the specific syndrome.
