摘  要:目的:探讨慢性阻塞性肺疾病患者肺功能指标与多层螺旋CT参数的相关性。方法:选取60例慢性阻塞性肺疾病患者为研究组,按照病情严重程度分为轻度组(n=18)、中度组(n=30)、重度组(n=12);另选40名同期体检健康志愿者为对照组。比较研究组和对照组、研究组不同病情严重程度患者的多层螺旋CT参数[(支气管壁面积百分比(WA%)、管壁厚度与体表面积的比值(T/BSA)、管腔面积(Ai)、管壁厚度与直径比(TDR)]及肺功能指标[肺容量(FVC)、第1秒用力呼气容积(FEV1)及FEV1/FVC],分析多层螺旋CT参数与肺功能指标的相关性及其对慢性阻塞性肺疾病的诊断价值。结果:研究组患者Ai、FVC、FEV1低于对照组(P<0.05);WA%、TDR、T/BSA、FEV1/FVC高于对照组(P<0.05)。不同病情严重程度患者FVC、FEV1、Ai有统计学差异(P<0.05),且轻度组>中度组>重度组;不同病情严重程度患者FEV1/FVC、T/BSA、WA%、TDR差异有统计学意义(P<0.05),且轻度组<中度组<重度组。相关性分析显示,WA%、TDR、T/BSA水平与FEV1/FVC均呈正相关关系(P<0.05),与FEV1、FVC均呈负相关关系(P<0.05);Ai水平与FEV1、FVC均呈正相关关系(P<0.05),与FEV1/FVC负相关(P<0.05)。ROC曲线分析显示,与WA%、Ai、T/BSA、TDR单项诊断相比,四项联合对慢性阻塞性肺疾病的诊断价值较高(P<0.05)。结论:慢性阻塞性肺疾病肺功能指标与多层螺旋CT参数有一定相关性;多层螺旋CT四项参数联合对慢性阻塞性肺疾病的诊断价值较高。Objective:To explore the correlation between multi-slice spiral CT parameters and pulmonary function indexes in patients with chronic obstructive pulmonary disease.Methods:A total of 60 patients with chronic obstructive pulmonary disease were selected as the study group,and divided into mild group(n=18),moderate group(n=30)and severe group(n=12)according to the severity of their condition,and 40 healthy patients were selected as the control group.The multi-slice spiral CT parameters[bronchial wall area percentage(WA%),tube wall thickness to body surface area ratio(T/BSA),lumen area(Ai),tube wall thickness to diameter ratio(TDR)]and lung function indicators[lung volume(FVC),forced expiratory volume at 1 second(FEV1),FEV1/FVC]were compared between those groups.The correlation between multi-slice spiral CT parameters and pulmonary function indexes and their diagnostic value for chronic obstructive pulmonary disease in each group were analyzed.Results:Compared with the control group,the levels of Ai,FVC and FEV1 in the study group decreased,and the levels of WA%,TDR,T/BSA and FEV1/FVC increased(P<0.05).The levels of FVC,FEV1 and Ai in the moderate and severe groups was lower than those in the mild group,and the moderate group was higher than the severe group.The levels of FEV1/FVC,T/BSA,WA%,and TDR in severe group were higher than those in moderate and mild groups(P<0.05),and the moderate was higher than the mild groups(P<0.05).Correlation analysis showed that the levels of WA%,TDR and t/BSA were positively correlated with FEV1/FVC and negatively correlated with FEV1 and FVC,while Ai levels were positively correlated with FEV1 and FVC and negatively correlated with FEV1/FVC(P<0.05).Compared with the single diagnosis of WA%,Ai,T/BSA and TDR,the four combinations had a higher diagnostic value for chronic obstructive pulmonary disease(P<0.05).Conclusion:Multi-slice spiral CT can improve the evaluation of chronic obstructive pulmonary disease of different severity,and has a certain correlation with FVC,FEV1,and FEV1/FVC,
