从叶天士虚劳“病-体-时”探析艾滋病相关性疲乏辨治思路

摘　　要：艾滋病相关性疲乏是在HIV感染后出现的一种以患者身心疲倦感为主要表现的疾病,在艾滋病人群中有较高的发病率,并且有重要的行为、心理及社会后果,而目前尚缺乏长期有效的药物治疗手段。本文根据该病的特点从叶天士虚劳病进行辨治,以“天人相应”思想为基础,分别从辨病、辨体、辨时三个角度进行阐发,按脾虚质和阳虚质调体固体、随四时病机与昼夜脏腑经气盛衰异治等方法论治该病,冀求能在抗反转录病毒疗法的基础上提高该病诊疗的特异性并长期稳定地改善艾滋病患者的疲乏症状,拓展艾滋病相关性疲乏的诊治思路。AIDS-related fatigue is a disease that occurs after HIV infection and is mainly characterized by physical and mental fatigue,with a high incidence in the AIDS population and important behavioral,psychological and social consequences.This paper identifies and treats the disease according to the characteristics of the syndrome from Ye Tianshi's deficiency disease,based on the thought of"correspondence between man and nature",and elaborates on it from three perspectives:disease differentiation,constitution differentiation and time differentiation.The disease is treated using methods such as regulating and strengthening constitution from the types of`Spleen-deficiency and Yang-deficiency,according to the four-season pathogenesis and the deficiency and excess of organ’s channel Qi during the day and night to differentiate the treatment.With the hope of improving the specificity of the treatment of this disease based on antiretroviral therapy and to alleviate the fatigue symptoms of AIDS patients in a stable and long-term manner,so as to expand the idea of diagnosis and treatment of AIIDS-related fatigue.
