“老十针”为主治疗脓毒症合并急性胃肠损伤的临床研究

摘　　要：目的 观察“老十针”配合常规药物治疗脓毒症合并急性胃肠损伤(acute gastrointestinal injury,AGI)的临床疗效。方法 将66例脓毒症合并AGI患者随机分为观察组和对照组,每组33例。对照组按照SEPSIS3.0治疗方案治疗,观察组在对照组基础上加用“老十针”针刺治疗。观察两组治疗前后肠鸣音分级、AGI分级、急性生理与慢性健康(acute physiology and chronic health evaluationⅡ, APACHEⅡ)评分及各项炎症指标[降钙素原(procalcitonin, PCT)、C-反应蛋白(C-reactive protein, CRP)]的变化情况。结果 两组治疗后肠鸣音分级及AGI分级与同组治疗前比较,差异均具有统计学意义(P<0.05)。观察组治疗后肠鸣音分级及AGI分级与对照组比较,差异具有统计学意义(P<0.05)。观察组治疗后APACHEⅡ评分及PCT水平较同组治疗前均显著下降,对照组治疗后APACHEⅡ评分及CRP、PCT水平较同组治疗前均显著下降,差异均具有统计学意义(P<0.05)。两组治疗后APACHEⅡ评分及CRP、PCT水平比较,差异均无统计学意义(P>0.05)。结论 “老十针”配合常规药物治疗可以改善脓毒症合并AGI患者的胃肠症状,促进胃肠道功能恢复。Objective To observe the clinical efficacy of“old ten needles”with conventional drugs in the treatment of sepsis complicated with acute gastrointestinal injury(AGI).Method A total of 66 patients with sepsis complicated with AGI were randomly divided into an observation group and a control group,33 cases in each group.The control group was treated according to the SEPSIS 3.0 treatment protocol,while the observation group was treated with“old ten needles”on the basis of the control group.Before and after the treatment,the bowel sound grading,AGI grading,acute physiology and chronic health evaluationⅡ(APACHEⅡ)scores and inflammatory indexes[procalcitonin(PCT),C-reactive protein(CRP)]were observed in both groups.Results After treatment,the differences of bowel sound grading and AGI grading of the two groups were statistically significant when compared with those of the same group before treatment(P<0.05).The difference of bowel sound grading and AGI grading between the treatment group and the control group after treatment was statistically significant(P<0.05).The APACHEⅡscore and PCT level of the treatment group decreased significantly compared with those of the control group,while the APACHEⅡscore,CRP and PCT level of the control group decreased significantly compared with those of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in APACHEⅡscore,CRP and PCT levels between the two groups after treatment(P>0.05).Conclusion“Old ten needles”combined with conventional medication therapy can improve the gastrointestinal symptoms and promote the recovery of gastrointestinal function in patients with sepsis complicated with AGI.
