摘 要:目的探讨补中益气汤联合地黄汤加减对肺肾气虚型慢性阻塞性肺疾病缓解期患者的临床疗效。方法116例患者随机分为对照组和观察组,每组58例,对照组给予常规治疗,观察组在对照组基础上加用补中益气汤联合地黄汤加减,疗程8周。检测临床疗效、IL-6、CX3CL1、MMP-9、MCP-1、a1-AT、营养指标(ALB、HGB、PALB)、肺功能指标(FEV1、FEV1/FVC)、血气指标(PaO_(2)、PaCO_(2))、中医证候评分、CAT评分变化。结果观察组总有效率高于对照组(P<0.05)。治疗后,2组IL-6、CX3CL1、MMP-9、MCP-1、a1-AT、PaCO_(2)、中医证候评分、CAT评分降低(P<0.05),营养指标、PaO_(2)、肺功能指标升高(P<0.05),以观察组更明显(P<0.05)。结论补中益气汤联合地黄汤加减可减轻肺肾气虚型慢性阻塞性肺疾病缓解期患者炎症反应,改善血气指标,纠正营养障碍,促进肺功能改善。AIM To explore the clinical effects of Modified Buzhong Yiqi Decoction combined with Dihuang Decoction on patients with chronic obstructive pulmonary disease in relief period due to Lung-Kidney Qi Deficiency.METHODS One hundred and sixteen patients were randomly assigned into control group(58 cases)for 8-week intervention of conventional treatment,and observation group(58 cases)for 8-week intervention of Modified Buzhong Yiqi Decoction,Dihuang Decoction and conventional treatment.The changes in clinical effects,IL-6,CX3CL1,MMP-9,MCP-1,a1-AT,nutritional indices(ALB,HGB,PALB),pulmonary function indices(FEV1,FEV1/FVC),blood gas indices(PaO_(2),PaCO_(2)),TCM symptom score and CAT score were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the two groups displayed decreased IL-6,CX3CL1,MMP-9,MCP-1,a1-AT,PaCO_(2),TCM symptom score,CAT score(P<0.05),and increased nutritional indices,PaO_(2),pulmonary function indices(P<0.05),especially for the observation group(P<0.05).CONCLUSION For the patients with chronic obstructive pulmonary disease in relief period due to Lung-Kidney Qi Deficiency,Modified Buzhong Yiqi Decoction combined with Dihuang Decoction can alleviate inflammatory responses,improve blood gas indices,correct nutritional disorders,and promote lung function improvement.
