当归四逆汤加味治疗冠状动脉粥样硬化性心脏病心绞痛临床疗效的Meta分析

摘　　要：目的:评价当归四逆汤加味治疗冠状动脉粥样硬化性心脏病(简称冠心病)心绞痛的临床疗效和安全性,为临床提供指导。方法:由两位研究员按照预先设定的纳入和排除标准系统检索中国知网、万方数据知识服务平台、中文科技期刊数据库(维普网)、中国生物医学文献数据库(CBM)、PubMed、Cochrane Library、EmBase等数据库中自建库至2023年1月收录的有关当归四逆汤加味治疗冠心病心绞痛的临床随机对照试验,并独立提取基础数据,采用Cochrane风险偏倚评估工具对所纳入文献进行质量评价,使用RevMan 5.4软件对纳入文献中的数据进行Meta分析。结果:共纳入13篇随机对照试验文章,均以中文发表。试验结果显示,与单独西医常规治疗相比,当归四逆汤加味联合常规西医治疗冠心病心绞痛的临床疗效好[相对危险度(RR)=1.26,95%置信区间(CI)[1.18,1.35],P<0.00001)],心电图疗效提高(RR=1.22,95%CI[1.08,1.38],P=0.001),心绞痛发作频率降低[均数差(MD)=-1.38,95%CI[-2.23,-0.42],P=0.005)],心绞痛发作持续时间缩短(MD=-1.38,95%CI[-2.34,-0.42],P=0.005),差异具有统计学意义。结论:当归四逆汤加味联合常规西药治疗冠心病心绞痛可提高整体临床疗效,且安全性好。Objective:To evaluate the clinical efficacy and safety of the Danggui Sini decoction(当归四逆汤)on coronary heart disease and angina pectoris,and to provide guidance for clinical practice.Methods:According to the preset inclusion and exclusion criteria,the two researchers searched the clinical randomized controlled trials(RCTs)of the Danggui Sini decoction for the treatment of coronary heart disease angina pectoris in the databases of CNKI,Wanfang,VIP,CBM,PubMed,Cochrane Library,EmBase from the time of establishment to January 2023,and independently extracted the basic data.The Cochrane evaluation manual handbook was used to evaluate the quality of the included literature,and Meta-analysis was performed on the extracted data by RevMan 5.4.Results:A total of 13 RCT studies were included,all of which were published in Chinese.The results showed that compared with conventional western medicine alone,the Danggui Sini decoction combined with conventional western medicine has a better clinical effect on coronary heart disease angina pectoris(RR=1.26,95%CI[1.18,1.35],P<0.00001).Electrocardiographic efficacy was improved(RR=1.22,95%CI[1.08,1.38],P=0.001),frequency of angina attacks was reduced(MD=-1.38,95%CI[-2.23,-0.42],P=0.005),duration of angina pectoris was shortened(MD=-1.38,95%CI[-2.34,-0.42],P=0.005),and the differences were statistically significant.Conclusion:The Danggui Sini decoction combined with conventional western medicine on coronary heart disease can improve the overall clinical efficacy and safety.
