基于数据挖掘探讨吕绍光治疗先兆流产的用药规律

摘　　要：目的:借助数据挖掘技术,结合临床经验探讨全国名中医吕绍光主任治疗先兆流产的用药规律,为临床治疗提供参考。方法:收集并整理出吕绍光主任治疗先兆流产的门诊病例处方,对纳入处方的中药名进行规范统一后采用Microsoft Excel2013建立数据集。通过古今医案云平台对处方所涉及中药的频次、四气、五味、归经及功效进行分析,接着再进行关联规则分析、聚类分析和复杂网络分析,深入挖掘处方中药数据规律。结果:本研究共纳入268首中药处方,涉及88种中药,出现频次居于前10位的为白术、山药、莲子、芡实、菟丝子、茯神、枸杞子、陈皮、竹茹和桑椹,药物功效以安胎、健脾、补肾为主,性味多为平、温,甘、苦、辛,多归脾经、肾经。关联规则分析出22个高频中药配伍,聚类分析出5类中药组方。复杂网络分析得出,白术、山药、莲子、芡实、菟丝子、茯神、枸杞子、陈皮、竹茹、桑椹和墨旱莲11味中药之间的关联程度最高。结论:吕绍光主任治疗先兆流产用药精简,治疗遵“脾肾同治”之法,脾气盛则肾气旺,冲任调和,加减化裁随证(症)治之,共奏安胎之效。Objective:To analyze the medication pattern of LYU Shaoguang,a famous Chinese medicine doctor in the treatment of threatened abortion,and to provide reference for clinical treatment.Methods:The cases prescriptions of threatened abortion from Director LYU Shaoguang were collected and sorted.After standardized and unified the names of Chinese medicine in the included prescription,the case data set was established by Microsoft Excel 2013.Firstly,data mining module of the Ancient and Modern Medical Case Cloud Platform was used to conduct frequency analysis of TCM drugs and their flavors,properties,meridians,and functions.Secondly,herbal group prescriptions were further analyzed through association rule analysis,cluster analysis and complex network analysis.Results:A total of 268 Chinese medicine prescriptions were included in this study,involving 88 types of herbal medicines.The top 10 herbs were Baizhu(Rhizoma Atractylodis Macrocephalae),Shanyao(Rhizoma Dioscoreae),Lianzi(Semen Nelumbinis),Qianshi(Semen Euryales),Tusizi(Semen Cuscutae),Fushen(Poria cum Radix Pini),Gouqizi(Fructus Lycii),Chenpi(Pericarpium Citri Reticulatae),Zhuru(Caulis Bambusae in Taenia)and Sangshen(Fructus Mori).The main effects of the herbal medicines are to calm the fetus,strengthen the spleen and tonify the kidneys.They are mostly moderate,warm,sweet,bitter or pungent.These drugs are mostly absorbed by spleen and kidney meridians.Twenty-two commonly used herbal combinations are identified by association rule analysis.Five herbal clusters are identified by cluster analysis.The results of complex network analysis showed that Baizhu,Shanyao,Lianzi,Qianshi,Tusizi,Fushen,Gouqizi,Chenpi,Zhuru,Sangshen and Mohanlian(Herba Ecliptae)had the highest correlation degree.Conclusion:In the treatment of threatened abortion,LYU Shaoguang follows the method of“treating the spleen and kidneys together”,with simplified medication to harmony Chong-Ren(冲任)meridians to prevent miscarriage.
