基于“态靶理论-调控炎性反应机制”探讨中医药防治脓毒症新策略

摘　　要：“态靶因果”辨证方略是仝小林院士将中医思维与网络药理学、蛋白质组学、代谢组学等现代分析方法相结合后提出的中医治疗疾病新理论。文章通过分析仝小林院士提出的“态靶因果”辨证方略,基于中医调控炎性反应机制,探讨中医药防治脓毒症的诊疗方法,以增强临床疗效,实现精准治疗。当下中医辨证认为脓毒症患者多分为三态:“毒态”“瘀态”“虚态”,此三“态”分别对应疾病不同的发展阶段,表现为病机的不同,通过辨证找准病机,结合患者临床表现、现代医学检查结果以及中药药理学,确定不同“态”所对应之靶方靶药,初步确立治则与治法,以实现“病”为纬,“态”为经,精准打“靶”,实现中医调态精准化。同时,中医诊疗要抓住疾病之“病因”,脓毒症的“因”无外乎外伤、内伤、情志,不同“因态”对应不同结果,临床症状差异多表现为气血、虚实的不同,掌握该病的气血、虚实变化,未病先预,既病防变。以脓毒症疾病之“果”寻找致病之“因”,审视疾病全过程、厘清疾病的发展阶段、归纳核心病机,最终确定理法方药,为脓毒症的中西医结合治疗提供新思路。The“state target cause and effect”strategy was proposed by Academician TONG Xiaolin.He combines Chinese medicine thinking with modern analytical methods such as network pharmacology,proteomics and metabolomics,thus proposing new theories of Chinese medicine in treating diseases.We analysed the dialectical strategy of“state-target cause and effect”proposed by academician TONG Xiaolin.Based on the mechanism of Chinese medicine regulating inflammatory response,the diagnostic and therapeutic methods of Chinese medicine was explored to prevent and treat sepsis,in order to enhance the clinical efficacy,so as to achieve precise treatment.Modern Chinese medicine scholars classify patients with sepsis into three states:“toxic state”,“stasis state”and“deficiency state”.These three“states”correspond to the different stages of development of the disease and are manifested in different disease mechanisms.By identifying the disease mechanism,combining the patient’s clinical manifestations,the results of modern medical examinations and the pharmacology of traditional Chinese medicine,we can determine the target formulas and medicines corresponding to the different“states”,and initially establish the rules and methods of treatment,in order to achieve“disease”as the weft,“state”as the warp,accurate hit“target”,so as to achieve the precision of Chinese medicine regulating state.At the same time,Chinese medicine diagnosis and treatment should capture the cause of the disease.The causes of sepsis are trauma,internal injuries,and emotions.Different causes correspond to different results.Differences in clinical symptoms are mostly manifested as differences in Qi(气)and blood,deficiencies and excesses.By grasping the changes in the Qi and blood of the patients,it is possible to anticipate the disease before it occurs and prevent it from occurring.Through the“outcone”of sepsis disease,the“cause”of the disease can be found by examining the whole process of the disease,clarifying the deve
