张玲基于玄府理论治疗寻常型银屑病经验撷精

摘　　要：银屑病是皮肤科难治性疾病之一,是一种与内外环境、个体基因与遗传、机体自身免疫等多种因素相关联的系统性疾病。本病对患者的身心健康造成了严重危害,目前尚无根治办法。中医治疗本病有独特优势,可在辨证论治的基础上有效控制皮损及其他症状,疗效确切且安全性高。玄府作为遍布人体周身的微小结构,是气血津液布散流转于人体的重要通路。玄府开阖有度,则气血津液正常输布,人体生理功能得以正常发挥;玄府开阖失司,则枢机不利,气血凝滞,神机失用,百病乃生。张玲教授在银屑病治疗方面具有丰富的临床经验,认为玄府郁闭,开阖失司,气血津液难以渗灌是银屑病迁延难愈、反复发作的病机,在治疗银屑病过程中注重开通玄府,恢复玄府功能,单独或综合运用祛邪开玄、治血开玄、补虚开玄的方法。具体治法包括疏风清热、解表散寒、清热利湿、清热凉血、理气活血、养血润肤、补火助阳、滋阴养液。治疗寻常型银屑病时结合以上治法,在祛邪扶正,透邪外出的基础上使玄府开阖有度,气血津液正常输布,皮损自然消退。文章追溯玄府理论,总结导师通过玄府理论辨治寻常型银屑病的临床体会及现代研究,另附验案一则,希冀为本病的治疗提供一定思路。Psoriasis is one of the dermatological diseases difficult to treat,which is a systemic disease associated with various factors such as internal and external environment,individual genes and genetics,and the body’s own immunity.This disease poses a serious threat to the physical and mental health of patients,and there is currently no cure.TCM has unique advantages in treating this disease,which can effectively control skin lesions and other symptoms on the basis of syndrome differentiation and treatment.The therapeutic effect is definite and the safety is high.Xuanfu(玄府),as a tiny structure scattered throughout the human body,is an important pathway for the distribution and circulation of Qi(气),blood,and body fluids in the human body.If the opening and closing of the Xuanfu are moderate,the normal distribution of Qi,blood,and body fluids will enable the normal functioning of the human body’s physiological functions.If the opening and closing of the Xuanfu are lost,the cardinal mechanism will be disadvantageous,Qi and blood will stagnate,divine mechanisms will be lost,and various diseases will arise.Professor ZHANG Ling has rich clinical experience in the treatment of psoriasis.The professor believes that the pathogenesis of psoriasis is characterized by the stagnation and closure of the Xuanfu area,loss of control over opening and closing,and difficulty in infiltrating Qi,blood,and body fluids.In the process of treating psoriasis,attention should be paid to opening up the Xuanfu,restoring its function,and using methods such as dispelling evil and opening the Xuanfu,treating blood and opening the Xuanfu,and supplementing deficiency and opening the Xuanfu alone or in combination.The specific treatment methods include dispelling wind and heat,relieving external cold,clearing heat and promoting dampness,clearing heat and cooling blood,regulating Qi and activating blood,nourishing blood and moisturizing the skin,supplementing fire and Yang(阳),nourishing Yin(阴)and nourishing fluids.When treating psoriasis
