摘 要:目的:回顾性比较常规手术与3D打印技术辅助手术治疗肱骨髁间骨折的疗效。方法:选择我科2016年10月至2022年10月收治的骨髁间骨折需行手术治疗的患者56例,分为3D打印模型组(26例)和对照组(30例)。3D打印模型组术前完成CT扫描后,提取数据通过3D打印机制作患者骨折部位实体模型。对照组术前完成CT扫描后进行三维重建,两者的手术原则和方法基本一致。比较两组患者手术时间、术中失血量、术前及术后3个月Mayo标准评分、术后关节活动度及骨折愈合时间。结果:3D打印模型组平均手术时间和术中失血量均显著少于对照组(P<0.05);手术前后Mayo肘关节评分:3D打印模型组分别为(43.6±5.8)分和(98.2±5.0)分,对照组分别为(44.3±5.5)分和(92.1±4.6)分,两组术后Mayo肘关节评分均较术前显著提高,术后3D打印模型组的Mayo肘关节评分显著高于对照组(P<0.05);术后3D打印模型组的优良率较对照组亦有所提高(P<0.05);3D打印模型组关节活动范围明显大于对照组(P<0.05);两组骨折愈合时间未见明显差异(P>0.05)。结论:肱骨髁间骨折手术治疗中应用3D打印实体模型,可有效降低手术难度,缩短手术时间,改善术后肘关节功能,临床上可取得更好的疗效。Objective:To retrospective compared the efficacy of conventional surgery and surgery assisted by 3D printing technology in the treatment of humeral intercondylar fractures.Methods:56 patients with humeral intercondylar fracture were enrolled in the study from October 2016 to October 2022.They were divided into two groups:26 cases of 3D printing group,30cases of conventional group.The individual models were used to simulate the surgical procedures and carry out the surgery according to plan.Operation duration,blood loss volume and time to fracture union were recorded.The final functional outcomeswerealso evaluated according the Mayo elbow-performance score,the motion of the elbow,were also evaluated.Results:The operation duration,blood loss volume for 3D printing group was(96.53±10.93)min and(140.3±30.65)ml;and for conventional group was(106.3±16.44)minand(186.6±47.22)ml,respectively.The Mayo elbow score before and after surgery was(43.6±5.8)and(98.2±5.0)in the 3D printinggroup and(44.3±5.5)and(92.1±4.6)in the control group,respectively.The Mayo elbow score of the 3D printing group was significantly higher than that of the control group(P<0.05).The range of motion in 3D printing group was significantly larger than that in control group(P<0.05).However,there was no significant difference in fracture healing time between the two groups(P>0.05).Conclusion:The application of 3D printed solid model in the operation of humeral intercondylar fracture can effectively reduce the difficulty of operation,shorten the operation time and improve the function of elbow joint after operation.
