三圣汤加减联合针刺对峡部裂性腰椎滑脱症患者术后疼痛及腰椎稳定性的影响

摘　　要：目的:研究探讨三圣汤加减联合针刺对峡部裂性腰椎滑脱症(IS)患者术后疼痛及腰椎稳定性的影响。方法:选自2020年6月-2023年6月80例IS患者,随机分为对照组和观察组各40例,两组均采取微创经椎间孔入路腰椎椎间体融合术(MIS-TLIF)治疗,对照组术后给予盐酸乙哌立松治疗,观察组术后给予三圣汤加减联合针刺治疗,比较两组证候积分、Oswestry功能障碍指数(ODI)、疼痛数字评分(NRS)、稳定性指标及临床疗效。结果:治疗4周后,两组证候积分、ODI指数、NRS评分皆有降低,观察组分值更低,组间有统计学意义(P<0.05);治疗4周后,两组稳定性指标皆有提高,观察组测量值提高更明显,组间有统计学意义(P<0.05);治疗4周后,观察组总有效率95.00%明显比对照组75.00%更高(P<0.05)。结论:三圣汤加减联合针刺对IS患者的疗效显著,优于单用西药治疗,可有助改善腰椎微创术后疼痛及其稳定性,有利于术后功能恢复。Objective:Research and exploration on the effect of Sansheng Decocton combined with acupuncture treatment on postoperative pain and lumbar stability in the treatment of isthmus type lumbar spondylolisthesis.Methods:Eighty patients with IS were selected from June 2020 to June 2023 and randomly divided into a control group and an observation group,with 40 cases in each group.Both groups were treated with minimally invasive surgery transforaminal lumbar interbody fusion(MIS-TLIF),while the control group was treated with epinephrine hydrochloride after surgery.The observation group was treated with modified Sansheng Decocton combined with acupuncture after surgery.The syndrome scores,Oswestry dysfunction index(ODI),pain numerical score(NRS),stability indicators,and clinical efficacy of the two groups were compared.Results:After 4 weeks of treatment,the syndrome score,ODI index,and NRS score of both groups decreased,and the observed component values were lower,with statistical significance between the groups(P<0.05);After 4 weeks of treatment,the stability indicators of both groups improved,and the measured values of the observation group showed a more significant improvement,with statistical significance between the groups(P<0.05);After 4 weeks of treatment,the total effective rate in the observation group reached 95.00%,significantly higher than the control group's 75.00%(P<0.05).Conclusion:The combination of modified Sansheng Decocton and acupuncture has a significant therapeutic effect on patients with IS,which is better than using Western medicine alone.It can help improve postoperative pain and lumbar stability after minimally invasive surgery,and is beneficial for postoperative functional recovery.
