基于剪切波弹性成像技术评价平乐郭氏正骨“筋滞骨错”手法对髌股关节炎治疗效果

摘　　要：目的:应用超声剪切波弹性成像技术测量髌股关节炎患者股内侧肌、股外侧肌的杨氏模量以及收缩比率在接受平乐郭氏正骨“筋滞骨错”手法治疗前后的差异,评价其对髌股关节炎的治疗效果。方法:自2019年11月-2020年11月,收治髌股关节炎患者54例,采用平乐郭氏正骨“筋滞骨错”手法治疗髌股关节炎组20天,每天2次,对比治疗前后股内侧肌、股外侧肌的杨氏模量、收缩比率以及疼痛视觉模拟评分及膝关节Lysholm评分差异。结果:髌股关节炎患者治疗后休息期以及收缩期股内侧肌、股外侧肌杨氏模量和收缩比率较治疗前均明显增大,差异具有统计学意义(P<0.05)。髌股关节炎患者治疗后疼痛视觉模拟评分降低,膝关节Lysholm评分增高,差异具有统计学意义(P<0.05)。结论:髌股关节炎患者伴有股四头肌痿软和收缩无力,采用“筋滞骨错”手法治疗髌股关节炎可以提高股内侧肌和股外侧肌收缩能力,减轻疼痛,改善膝关节功能。Objective:The purpose is to apply the ultrasonic shear wave elastic elastography to measure the differences of Young's modulus and contraction ratio of the medial and lateral quadricep in patients with patellofemoral arthritis,before and after the treatment of Pingle Guo's orthopedic method of“Jin zhi gu cuo”Tuina,which is used to evaluate its therapeutic effect on patellofemoral arthritis.Methods:From November 2019 to November 2020,54 cases of patellofemoral arthritis were treated with Pingle Guo's orthopedic method of“Jin zhi gu cuo”Tuina in the patellofemoral arthritis group for 20 days,twice daily.The differences were compared before and after treatment in terms of Young's modulus of the medial and lateral muscles of the quadricep,contraction ratio,visual analog score,and knee joint Lysholm score.Results:The Young's modulus and contraction ratio of medial and lateral muscles of the quadricep when resting and contracting in patients with patellofemoral arthritis increased significantly after the treatment(P<0.05).The patient's pain visual analog score was reduced after the treatment and knee joint Lysholm score increased(P<0.05).Conclusion:Medial and lateral muscles of the quadricep with patellofemoral arthritis are weak and powerless in contraction.Pingle Guo's orthopedic method of“Jin zhi gu cuo”Tuina can improve the contraction ability,relieve pain,and improve knee joint function.
