基于大鱼际隆起尺寸选择儿童I-gel喉罩规格的可行性研究

摘　　要：目的 评价基于大鱼际隆起尺寸选择儿童I-gel喉罩规格的可行性。方法 选取2022年10月至2024年1月于医院接受全身麻醉外科手术的120例患儿作为研究对象,采用随机数字表法分为对照组和观察组,每组60例。对照组根据制造商提供的体质量-规格表选择I-gel喉罩规格,观察组根据患儿大鱼际隆起尺寸选择I-gel喉罩规格。比较两组首次置入成功率、置入时间、麻醉中调整率、漏气率、术后并发症,以及两组喉罩置入前、喉罩置入后5 min、手术30 min后、手术结束前5 min和拔除喉罩后的血氧饱和度(SpO_(2))、呼吸末CO_(2)分压(Pet CO_(2))。结果 两组喉罩首次置入成功率、置入时间、麻醉中调整率和漏气率比较,差异无统计学意义(P>0.05)。两组各时点SpO_(2)、PetCO_(2)比较,差异无统计学意义(P>0.05)。两组喉罩血染、咽喉疼痛、声音嘶哑、恶心呕吐和吞咽困难等并发症发生率比较,差异无统计学意义(P>0.05)。结论 基于大鱼际隆起尺寸选择儿童I-gel喉罩规格方法的可靠性与基于体质量选择方法相近,为紧急情况下无法获知体质量参数的患儿提供一种新的喉罩规格选择方法。Objective To evaluate the feasibility of selecting the size of children's I-gel laryngeal mask based on the size of the large thenar bulge.Methods A total of 120 children who underwent general anesthesia surgery in the Department of Pediatric Surgery of our hospital from October 2022 to January 2024 were included as study objects.According to random number table method,the children were divided into control group and study group,with 60 cases in each group.The control group selected the I-gel laryngeal mask model according to the weight-specification table provided by the manufacturer,and the study group matched the laryngeal mask specifications with the most similar long and wide diameters according to the large thenar length and width.The first implantation success rate,implantation time,adjustment rate during anesthesia,air leakage rate and postoperative complications were compared between groups.The differences of blood oxygen saturation(SpO_(2)),end-respiratory CO_(2) partial pressure(PetCO_(2))and airway peak pressure(Ppeak)were recorded before laryngeal mask placement,5 min after laryngeal mask placement,30 min after surgery,5 min before surgery and after laryngeal mask removal.Results There were no significant differences in the success rate,time,adjustment rate during anesthesia and air leakage rate between the two groups(P>0.05).There was no significant difference in SpO_(2),PetCO_(2) between the two groups at each time point(P>0.05).There was no significant difference in the incidence of complications such as blood staining of laryngeal mask,sore throat,hoarseness,nausea,vomiting and dysphagia among groups(P>0.05).Conclusion The reliability of the I-gel laryngeal mask selection method based on the size of the large thenar bulge is similar to that based on the weight selection method,which provides a new method for the selection of laryngeal mask specifications for children who cannot know the weight parameters in emergency situations.
