手掌色络诊法与糖尿病足病临床实践

摘　　要：糖尿病足病临床证候繁杂,诊断时难以立时判别,需用更多的诊断方法增加辨识。乔凯明副主任医师结合《黄帝内经》络脉诊法、五色诊法及《望诊遵经》“望色十法”,总结出手掌色络诊法,手掌色络诊法是依托手掌皮色及络色以综合辨识疾病病位、病性的诊断方法,糖尿病足病急性期手掌色络诊多见“大、小鱼际络赤,大鱼际色赤、色抟聚(提示胃肠道积热),指腹色赤”;糖尿病足病慢性期手掌色络诊多见“大鱼际青或大鱼际上赤下青,小鱼际络赤,主动脉区到心区色青,指腹色赤”。此法据多年临床反复应用取得了较好效果,可用以襄助脉管疾病的中医临床诊疗。The clinical syndromes of diabetic foot disease are complicated,and it is difficult to distinguish immediately in diagnosis,so we need to use more diagnostic methods to increase identification.Associate Chief Physician Qiao Kaiming summarized the palm color collateral diagnosis method by combining the Inner Canon of the Yellow Emperor collateral diagnosis method,the“five-color diagnosis method”and the“ten methods of color observation”.The palm color collateral diagnosis method is a diagnostic method to comprehensively identify the disease location and disease nature by relying on the skin color and collateral color of the palm.In the acute stage of diabetic foot,the diagnosis of palm color collateral is more common:“thenar and hypothenar collateral red,thenar red,color kneading(indicating gastrointestinal heat accumulation),finger abdomen red”;Chronic palm color collateral diagnosis more commonly seen“thenar blue or thenar upper part blue and lower part red,hypothenar collateral red,aortic area to heart area blue,finger abdomen red.”This method has been applied repeatedly for many years and has achieved good results.It can be used to assist in the clinical diagnosis and treatment of vascular diseases.
