某三甲综合医院2015年-2022年日间手术中心老年患者的疾病特征分析

摘　　要：目的分析某三甲综合医院日间手术中心2015年-2022年老年患者的病例数据,为科学规范日间手术中心老年患者的管理提供依据。方法收集2015年1月1日-2022年12月31日该院日间手术中心集中收治的≥60岁患者的病例资料,统计手术基本情况、手术术式变化、麻醉方式变化、合并慢性疾病情况、延迟出院率和转专科病房率。结果8年内,共有6500例老年患者接受日间手术(男4010例、女2490例)。平均年龄为67±5.53岁,最大年龄为91岁。其中,60岁~69岁患者为4780人(73.53%)。2015年至2019年,住院人数呈线性增长趋势,年均增长率26.14%,合并慢性疾病的病例数,从2015年20.79%增至2020年66.48%;三级手术从2015年8.29%增至2022年的30.02%,四级手术从2015年1.17%增至2022年13.96%;全麻手术,从2015年的211例(43.87%)增至2021年的554例(69.69%);术后延迟出院为41例(0.63%),术后转专科病房34例(0.52%)。结论老年患者日间手术呈现数量增加,难度升高的趋势。日间手术的管理模式同样适合推荐给老年患者。Objectives The aim of this study is to analyze the case data of elderly patients treated at the day surgery center of a tertiary general hospital from 2015 to 2022.The goal is to provide a basis for the scientific and standardized management of elderly patients undergoing day surgery.Methods Case data of patients aged≥60 years old who were admitted to the day surgery center of this hospital from January 1,2015 to December 31,2022 were collected.The basic situation of surgery,changes in surgical procedures,changes in anesthesia methods,combined chronic diseases,delayed discharge rate and transfer rate to specialist wards were analyzed.Results Within 8 years,a total of 6500 elderly patients(4010 males and 2490 females)received day surgery.The mean age was 67±5.53 years,with a maximum age of 91 years.Among them,4780 patients(73.53%)were aged 60-69 years.From 2015 to 2019,the number of hospitalizations showed a linear growth trend,with an average annual growth rate of 26.14%.The number of cases combined with chronic diseases increased from 20.79%in 2015 to 66.48%in 2020.Level 3 surgery increased from 8.29%in 2015 to 30.02%in 2022,and level 4 surgery increased from 1.17%in 2015 to 13.96%in 2022.General anesthesia procedures increased from 211(43.87%)in 2015 to 554(69.69%)in 2021.The delayed discharge rate was 41 cases(0.63%),and the postoperative transfer rate was 34 cases(0.52%).Conclusions Day surgeries for elderly patients are increasing in number and difficulty.The day surgery management model is also recommended for older patients.
