态靶辨证在胃肠实热型糖尿病合并高脂血症中的应用:基于大黄黄连泻心汤加红曲、生山楂

摘　　要：糖尿病是一种临床常见病,合并症很多,其中合并高脂血症较为常见。糖尿病合并高脂血症属中医“消渴”合并“痰证”“浊证”“瘀证”等范畴。仝小林院士基于多年临床研究把糖尿病的发展过程分为郁、热、虚、损4个阶段,胃肠实热为“热”阶段的常见证型,临床症状多见脘腹胀满、口干、口渴、口臭、大便秘结、舌红、苔黄燥、脉沉实等。此类患者以热为态,治疗需清热通腑以调态,主方选用大黄黄连泻心汤;若合并高脂血症,以痰浊为靶,用红曲、生山楂化浊降脂以打靶。针对胃肠实热型糖尿病合并高脂血症,运用态靶辨证既能调患者实热之态,又可对痰浊症状精准打靶。Nowadays,as a common clinical disease,diabetes has many complications,including hyperlipidemia.Diabetes combined with hyperlipidemia belongs to the category of diabetes combined with“phlegm syndrome”,“turbid syndrome”,“blood stasis syndrome”,etc.Academician Tong Xiaolin,based on years of clinical research,divided the development process of diabetes into four stages:depression,heat,deficiency,and damage.The excess heat in the stomach and intestines is the common syndrome type in the“heat”stage.The clinical symptoms are more common:abdominal distention,dry mouth,thirst,bad breath,constipation,red tongue,dry fur,and dull pulse.This type of patient is characterized by heat,and treatment requires clearing heat and clearing the internal organs to regulate their state.The main formula is Dahuang Huanglian Xiexin Decoction.If combined with hyperlipidemia,phlegm turbidity should be targeted,and Monascus and Raw Hawthorn should be used to reduce turbidity and blood lipids.For gastrointestinal excess heat type diabetes with hyperlipidemia,the use of state target syndrome differentiation can not only regulate the state of excess heat,but also accurately target the symptoms of phlegm turbidity.
