成人使用一次性包皮缝合器行包皮环切术后吻合钉脱落影响因素分析

摘　　要：目的:分析成人使用一次性包皮缝合器行包皮环切术后吻合钉脱落影响因素。方法:回顾性分析上海大学附属四一一医院泌尿外科2020年1月至2023年9月采用一次性包皮缝合器行包皮环切术患者625例,根据是否自行脱钉,分为自行脱钉者558例和需医师辅助脱钉67例。单因素和多因素Logistic分析吻合钉脱落影响因素。结果:多因素logistic分析结果表明年龄(OR=5.587,95%CI:1.922~15.062;P=0.000)、洗浴频次(OR=1.538,95%:1.062~2.225;P=0.023)、晨勃频次(OR=5.157,95%CI:1.344~13.758;P=0.001)、术前性生活频次(OR=5.169,95%CI:1.348~13.774;P=0.001)、性欲(OR=2.318,95%CI:1.156~4.648;P=0.018)、糖尿病(OR=2.587,95%CI:1.321~5.062;P=0.006)及龟头炎(OR=2.572,95%CI:1.308~5.126;P=0.008)是一次性包皮缝合器脱钉的独立影响因素。结论:年龄、洗浴频次、晨勃频次、术前性生活频次、性欲、糖尿病及龟头炎是一次性包皮缝合器脱钉的主要影响因素。Objective:To analyze the factors influencing the detachment of anastomotic staples after circumcision using a disposable circumcision stapler in adults.Methods:A retrospective analysis was performed on 625 patients undergoing circumcision with disposable foreskin sutures from January 2020 to September 2023 in the Department of Urology,41l Hospital Affiliated to Shanghai University.According to whether or not removed nailings by themselves,they were divided into 558 patients who removed nailings by themselves and 67 patients who required physician assistance.Univariate and multivariate logistic analysis were performed to analyze the influencing factors of anastomotic nail shedding.Results:Multivariate logistic analysis showed that age(OR=5.587,95%CI:1.922-15.062;P=0.000),bath frequency(OR=1.538,95%CI:1.062-2.225;P=0.023),morning erection frequency(OR=5.157,95%CI:1.344-13.758;P=0.001),preoperative sexual activityfrequency(OR=5.169,95%CI:1.348-13.774;P=0.001),libido(OR=2.318,95%CI:1.156-4.648;P=0.018),diabetesmellitus(OR=2.587,95%CI:1.321-5.062;P=0.006)and balanitis(OR=2.572,95%CI:1.308-5.126;P=0.008)were independent risk factors of anastomotic nail shedding after circumcision with disposable foreskin suture(P<0.05).Conclusion:Age,bath frequency,morning erection frequency,preoperative sexual activity frequency,libido,diabetes mellitus and balanitis are the main rinfluencing factors for staple detachment when using a disposable circumcision stapler.
