摘　　要：目的:探讨氨甲环酸对老年全膝关节置换术患者术后康复效果的影响。方法:回顾性分析2019年3月—2021年7月本院骨科收治的92例老年全膝关节置换术患者作为研究对象。依据治疗方法不同将患者分为治疗组(n=48)和对照组(n=44)。对照组给予常规治疗,治疗组在切口闭合前静脉滴注氨甲环酸,比较两组输血率、输血量、术后失血量、隐性失血量、血红蛋白(Hb)、红细胞计数(RBC)、自觉疼痛评分、关节周径及并发症发生率。结果:治疗组输血率、输血量、术后失血量和隐性失血量均低于对照组(P<0.05);治疗组术后Hb、RBC均高于对照组(P<0.05);治疗组术后7 d和14 d的自觉疼痛评分和关节周径均低于对照组(P<0.05);治疗组并发症发生率低于对照组(P<0.05)。结论:在老年全膝关节置换术后使用氨甲环酸能够控制患者的术后失血量,降低疼痛感,促进关节功能的恢复,且安全性较高。Objective:To investigate the effect of tranexamic acid on postoperative rehabilitation in elderly patients undergoing total knee arthroplasty.Methods:A retrospective analysis of 92 elderly patients with total knee arthroplasty admitted to the department of orthopedics of our hospital from March 2019 to July 2021 was conducted as the research object.According to different treatment methods,the patients were divided into treatment group(n=48)and control group(n=44).The control group was given routine treatment,and the treatment group was given intravenous infusion of tranexamic acid before incision closure.The blood transfusion rate,blood transfusion volume,postoperative blood loss,hidden blood loss,hemoglobin(Hb),red blood cell count(RBC),subjective pain score,joint circumference and complication rate were compared between the two groups.Results:The blood transfusion rate,blood transfusion volume,postoperative blood loss and hidden blood loss in the treatment group were lower than those in the control group(P<0.05).The postoperative Hb and RBC in the treatment group were higher than those in the control group(P<0.05).The pain score and joint circumference of the treatment group were lower than those of the control group at 7 d and 14 d after operation(P<0.05).The incidence of complications in the treatment group was lower than that in the control group(P<0.05).Conclusion:The use of tranexamic acid after total knee arthroplasty in the elderly can control the postoperative blood loss of patients,reduce pain,promote the recovery of joint function,and have high safety.
