通窍鼻炎颗粒辅助布地奈德喷鼻治疗慢性鼻窦炎伴鼻息肉内镜术后患者的效果

摘　　要：目的:探究通窍鼻炎颗粒联合布地奈德喷鼻对慢性鼻窦炎伴鼻息肉内镜术后鼻功能恢复的影响。方法:选取2022年1月—2023年7月本院诊治的慢性鼻窦炎伴鼻息肉患者86例,按照治疗方案不同分为观察组(43例)和对照组(43例),两组均接受鼻内镜术治疗,术后对照组使用布地奈德喷鼻治疗,观察组在对照组基础上使用通窍鼻炎颗粒,均治疗2个月。比较两组鼻通气功能[鼻腔最小横截面积(nasal minimum cross-sectional area,NMCA)、鼻腔容积(nasal cavity volume,NCV)、鼻腔最小横截面积所处的位置到前鼻孔的距离(distance of the nasal minimal cross-sectional area from the nostril,DCAN)、鼻气道阻力]、Lund-Kennedy评分、致炎因子、临床疗效及不良反应。结果:治疗后,观察组NMCA、NCV均大于对照组,DCAN和鼻气道阻力均小于对照组(P<0.05);观察组息肉、水肿、鼻漏、瘢痕、结痂得分低于对照组(P<0.05);观察组白细胞介素-2、C反应蛋白、白细胞介素-6、水平低于对照组(P<0.05);观察组总有效率为95.35%,高于对照组的81.40%(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:于慢性鼻窦炎伴鼻息肉内镜术后,采用通窍鼻炎颗粒联合布地奈德喷鼻方案的疗效明显,能有效改善患者鼻通气功能及鼻黏膜形态,减轻炎症,且应用安全性较高。Objective:To explore the effect of Tongqiao Biyan granules combined with budesonide nasal spray on nasal function recovery after endoscopic sinus surgery for chronic sinusitis with nasal polyps.Methods:A total of 86 patients with chronic sinusitis and nasal polyps diagnosed and treated in our hospital from January 2022 to July 2023 were selected and divided into observation group(43 patients)and control group(43 patients)according to different treatment schemes.Both groups were treated with endoscopic sinus surgery.After operation,the control group was treated with budesonide nasal spray,and the observation group was treated with Tongqiao Biyan granules on the basis of the control group.Both groups were treated for 2 months.Nasal ventilation function[nasal minimum cross-sectional area(NMCA),nasal cavity volume(NCV)],distance of the nasal minimal cross-sectional area from the nostril(DCAN),nasal airway resistance J,Lund-Kennedy score,inflammatory factors,clinical efficacy and adverse reactions were compared between the two groups.Results:After treatment,NMCA and NCV in the observation group were higher than those in the control group,and DCAN and nasal airway resistance were lower than those in the control group(P<0.05).The scores of polyps,edema,rhinorrhea,scar and scab in the observation group were lower than those in the control group(P<0.05).The levels of interleukin-2,C-reactive protein and interleukin-6 in the observation group were lower than those in the control group(P<0.05).The total effective rate of the observation group was 95.35%,which was higher than 81.40% of the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:After endoscopic sinus surgery for chronic sinusitis with nasal polyps,the curative effect of Tongqiao Biyan granules combined with budesonide nasal spray is obvious,which can effectively improve the nasal ventilation function and nasal mucosa morphology of patients,reduce inflammation,and has high
