综合护理在高出血风险重症患者行局部枸橼酸抗凝连续肾脏替代疗法中的应用效果

摘　　要：目的探讨在高出血风险重症患者行局部枸橼酸抗凝连续肾脏替代疗法(CRRT)中综合护理的应用价值。方法研究时间为2021年6月至2023年8月,研究对象为63例行局部枸橼酸抗凝CRRT治疗的高出血风险重症患者,根据随机数字表法分组,对照组31例,实施常规护理,观察组32例,实施综合护理,比较两组血钾(K^(+))、血钙(Ca^(2+))水平等变化情况。结果两组干预后K^(+)、Ca^(2+)水平较干预前下降,且干预后观察组K^(+)、Ca^(2+)水平更低(P<0.05);两组干预后健康调查简表(SF-36)评分较干预前提高,且干预后观察组SF-36评分更高(P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论有着高出血风险的重症CRRT患者应用综合护理可改善治疗效果,降低并发症发生风险,促进生活质量提高。Objective To investigate the application value of integrated nursing care in local citrate anticoagulation continuous renal replacement therapy(CRRT)in critically ill patients with high bleeding risk.Methods The study period was from June 2021 to August 2023,and the study subjects were 63 cases of local citrate anticoagulation CRRT treatment in critically ill patients with high bleeding risk,grouped according to the randomized numerical table method,with 31 cases in the control group,implementing routine care,and 32 cases in the observation group,implementing integrated care,comparing the changes in blood potassium(K^(+))and blood calcium(Ca^(2+))levels and other changes in the two groups.Results The levels of K^(+)and Ca^(2+)in the two groups decreased after intervention,and the levels of K^(+)and Ca^(2+)in the observation group were lower after intervention(P<0.05).The SF-36 score of the two groups after intervention was higher than that before intervention,and the SF-36 score of the observation group was higher after intervention(P<0.05).The complication rate of observation group was lower than that of control group(P<0.05).Conclusions The application of comprehensive nursing care to critically ill CRRT patients with high risk of hemorrhage can improve the therapeutic effect,reduce the risk of complications,and promote the improvement of quality of life.
