摘  要:目的:探讨改良小切口后外侧入路在全髋关节置换术中的应用。方法:选取2020年1月~2023年10月期间,在我院行全髋关节置换术的82例患者的临床资料进行研究。采用随机数表法分为传统组(传统后外侧入路,n=41)和改良组(改良小切口后外侧入路,n=41)。比较两组患者围术期指标、血清创伤应激因子[血清皮质醇(Cor)、肾上腺素(AD)、去甲肾上腺素(NE)]及并发症发生率的差异。结果:改良组患者手术时间、术中出血量、切口长度及住院时间等围术期指标均优于传统组(P<0.05);两组患者Cor、AD、NE等应激反应均高于同组手术前,且改良组低于传统组(P<0.05);改良组(7.32%)和传统组(17.07%)的并发症发生率未见显著差异(P>0.05)。结论:改良小切口后外侧入路在全髋关节置换术中展现出了显著的优势,优化了围术期指标,降低了患者术后的应激反应。Objective:To explore the application of modified small incision posterolateral approach in total hip arthroplasty.Methods:The clinical data were collected from 82 patients undergoing total hip arthroplasty in the hospital between January 2020 and October 2023.According to random number table method,patients were divided into traditional group(traditional posterolateral approach,n=41)and modified group(modified small incision posterolateral approach,n=41).The differences in perioperative indexes,serum traumatic stress factors[serum cortisol(Cor),adrenaline(AD),norepinephrine(NE)]and incidence of complications between the two groups were compared.Results:The perioperative indexes(operation time,intraoperative blood loss,incision length,length of hospital stay)in modified group were better than those in traditional group(P<0.05).After surgery,Cor,AD and NE were increased in both groups,which were lower in modified group than traditional group(P<0.05).There was no significant difference in the incidence of complications between modified group and traditional group(7.32%vs 17.07%,P>0.05).Conclusion:Modified small incision posterolateral approach shows significant advantages in total hip arthroplasty,which can optimize perioperative indexes and reduce postoperative stress response.
