摘 要:目的:采用序贯法测定复合阿芬太尼时环泊酚抑制日间乳腺微创手术体动反应的半数有效剂量(ED 50)。方法:选择2023年10月至2024年1月于日间手术中心行乳腺微创旋切术的患者,年龄18~65岁,BMI 18.5~25.0 kg/m 2,ASA分级Ⅰ~Ⅱ级。静脉注射阿芬太尼10μg/kg,30 s后静脉注射环泊酚,待患者睫毛反射消失后开始手术。术中体动反应≥2级为阳性。采用Dixon序贯法试验,环泊酚初始剂量0.40 mg/kg,剂量梯度为0.04 mg/kg,采用Probit回归分析法计算环泊酚抑制体动反应的ED 50和ED 95。结果:共纳入36例患者。复合10μg/kg阿芬太尼时环泊酚抑制日间乳腺微创手术体动反应的ED 50(95%CI)为0.285(0.263~0.303)mg/kg;ED 95(95%CI)为0.337(0.314~0.421)mg/kg。结论:复合10μg/kg阿芬太尼时环泊酚抑制日间乳腺微创手术体动反应的ED 50为0.285 mg/kg。Aim:To determine the median effective dose(ED 50)of ciprofol combined with alfentanil for inhibiting body motor response during minimally invasive breast surgery using a sequential method.Methods:Patients undergoing minimally invasive breast tumor resection in a daytime surgery center from October 2023 to January 2024 were selected.The patients′ages ranged from 18 to 65 years,BMI ranged from 18.5 to 25.0 kg/m 2,and ASA classification ranged from gradeⅠto gradeⅡ.Alfentanil was administered intravenously at a dose of 10μg/kg,followed by ciprofol being given intravenously after 30 seconds.Surgery commenced once the patients′eyelash reflex disappeared.Any patient displaying movement response≥grade 2 during the procedure was considered to have a positive response.Dixon sequential method was employed,starting ciprofol at an initial dose of 0.40 mg/kg with an adjacent dose difference of 0.04 mg/kg.The ED 50 and ED 95 of ciprofol in suppressing body motor response were calculated using Probit regression analysis.Results:A total of 36 patients were included.The ED 50(95%CI)of ciprofol in suppressing body motor response during daytime minimally invasive breast surgery combined with 10μg/kg alfentanil was 0.285(0.263-0.303)mg/kg,and the ED 95(95%CI)was 0.337(0.314-0.421)mg/kg.Conclusion:The ED 50 of ciprofol in suppressing body motor response during daytime minimally invasive breast surgery combined with 10μg/kg alfentanil was 0.285 mg/kg.
