摘 要:碳青霉烯类抗菌药是治疗铜绿假单胞菌(PA)等难治性革兰阴性杆菌的有效药物,但该类药物在临床的广泛及不规范的使用导致耐碳青霉烯类PA(CRPA)逐渐增多,使得PA导致的临床感染性疾病的治疗成为难题。本文从产生药物灭活酶、细胞外膜通透性改变、细菌主动外排及生物膜形成等方面对CRPA的耐药机制进行综述,以期对临床合理用药以及防止耐药菌的产生和传播提供帮助。Carbapenem antibiotics are effective drugs for treating multidrug-resistant Gram-negative bacteria such as Pseudomonas aeruginosa(PA).However,the widespread and improper use of these drugs in clinical settings has led to an increase in carbapenem-resistant PA strains,posing a challenge to the treatment of clinical infections caused by this bacterium.This article reviews the resistance mechanism of CRPA from the aspects of drug-inactivating enzyme production,extracellular membrane permeability change,bacterial active efflux and biofilm formation,in order to provide help for clinical rational drug use and prevention of the generation and spread of drug-resistant bacteria.
