基于《黄帝内经》天人合一观念探讨寒冷刺激对脑卒中发病的影响及中医预防性干预

摘　　要：脑卒中严重威胁人类的健康和生命,寒冷刺激是诱发脑卒中疾病的重要因素之一,《黄帝内经》中提及“夫人生于天地,悬命于天,天地合气,命之曰人”“夫百病之始生也,皆生于风雨寒暑”,揭示了人体与自然界直接存在协调统一的内在联系。该文基于《黄帝内经》天人合一观念,对寒冷刺激与脑卒中发病的关系、内在机制以及可能的预防措施等方面进行综述。寒邪侵袭机体后易伤及阳气,气血运行阻滞,经脉失养,气滞血瘀,以致机体产生疾病。现有证据提示寒冷刺激与脑卒中发病和不良预后呈正相关,并且影响具有累积效应和滞后效应,其主要机制为诱发血管收缩、动脉痉挛,加剧凝血功能障碍、改变血液流变学特征增加血液黏稠度,诱发炎性反应和血管内皮损伤。基于“天人合一”观念,卒中发病及高危人群应基于地域特点和自身体质,实时关注气象状态变化,及时进行个人健康状态管理,降低卒中发作和不良预后风险。Stroke is a serious threat to human health and life,cold stimulation is one of the important factors inducing stroke disease,Inner Canon of Huangdi mentioned“man is born on the earth and is en-dowed with life by the heavens,owing to the integration of the Tianqi and Diqi,man comes into existence”,“all kinds of diseases are born in wind,rain,cold and heat”,revealing the human body and nature directly coordinated and unified internal connection.Based on the concept of harmony between nature and man in Inner Canon of Huangdi,this paper reviews the relationship,internal mechanism and possible preventive measures between cold stimulation and stroke.After the cold evil attacks the body,it is easy to injure Yang,block the movement of Qi and blood,lose the meridians,and cause the body to produce diseases.Existing evidence suggests that cold stimulation is positively correlated with the incidence and adverse prognosis of stroke,and its effects have cumulative and delayed effects,the main mechanisms of which are inducing vasoconstriction and arterial spasm,aggravating coagulation dysfunction,changing hemorheological characteristics,increasing blood viscosity,inducing inflammatory response and vascular endothelial injury.Based on the concept of“harmony between nature and man”,stroke incidence and high-risk groups should pay real-time attention to the transformation of meteorological status based on regional characteristics and their own physique,and timely manage personal health status to reduce the risk of stroke onset and poor prognosis.
