基于“一气周流”理论探讨帕金森病病机及论治

摘　　要：清代名医黄元御精读中国古代医学经典和自然哲学著作,在“天人相应”和“气一元论”的基础上提出了“土枢四象,一气周流”的学术理论,呈现为左升右降、中焦斡旋的气机运转模式,“中焦为轴,四象为轮”,枢轴运转,则阴平阳秘,脏腑调和。帕金森病(Parkinson's disease,PD)作为全球第二大神经系统退行性病变,病情呈缓慢性进展,现代医学对本病暂无有效的治疗方法,探索新的治疗方法意义重大。该文将运用黄元御的“一气周流”理论,从整体动态的角度去探讨帕金森病的病机及治则,提出帕金森病的病机以中土失运为根本,乙木下陷、肾水下寒、肺金右滞为关键,共同引起全身气机周流阻滞;并提出了运用黄芽汤加减健运脾胃、斡旋中焦,桂枝苓胶汤加减疏肝解郁、清风泄热,天魂汤加减温肾助阳、燥湿解郁,下气汤加减收敛肺气、调达肝气,以达到恢复全身气机周转的目的,从而为帕金森病提供新的治疗思路。HUANG Yuanyu,a famous doctor in the Qing Dynasty,studied the ancient Chinese medical classics and natural philosophy works,and put forward the academic theory of“Qi circling in round,the earth is the pivot of the other four Zang-organs”based on the theories of“correspondence between human body and natural environment”and“unitary theory”.It exhibits a pattern of Qi activity operation with the left side rising,the right side falling and the middle Jiao rotating.The central earth as the axis surrounded by the four elements in the visceral aspect.If Qi activity is functioning normally,Yin will being calm and Yang will being sound,Zang and Fu organs will being harmonious.Parkinson's disease(PD),as the second largest neurodegenerative disease in the world,has a slow progression,and there is no effective treatment for this disease in modern medicine,so it is of great significance to explore new treatment methods.In this paper,we will apply HUANG Yuanyu's theory of“Qi circling in round”,exploring the pathological mechanism and therapeutic principle of Parkinson's disease from a holistic and dynamic perspective.It is proposed that the pathological mechanism of Parkinson's disease is based on the loss of movement of the central earth as the root,the sinking of the ethyl wood,the coldness under the water of the kidney,and the right stagnation of the gold of the lung as the keys.The above pathological mechanism mentioned creates a blockage of Qi activity together.It also proposes the use of Huangya Decoction(黄芽汤)with subtractions to strengthen the spleen and stomach and mediate the middle Jiao;Guizhi Lingjiao Decoction(桂枝苓胶汤)with subtractions to ease the liver and relieve depression,clear the wind and drain the heat;Tianhun Decoction(天魂汤)with subtractions to warm the kidneys and boost the Yang,dry dampness and relieve depression;Xiaqi Decoction(下气汤)with subtractions to astringe lung Qi and regulate liver Qi;in order to achieve the purpose of restoring Qi turnover in the whole body,
