摘 要:目的分析同型半胱氨酸(Hcy)、尿微量清蛋白/肌酐比值(ACR)联合胱抑素(Cys-C)诊断高血压早期肾损害的诊断价值。方法选取嘉峪关市中医医院2021年5月-2023年9月期间收治的83例原发性高血压患者为研究组,根据高血压分级标准将其分为1级24例、2级32例和3级27例,另选取83例同期行健康体检的老年人为健康组,均进行血清Hcy、尿ACR和血清Cys-C检测,比较各组指标表达水平;同时统计83例原发性高血压患者中的早期肾损害发生情况,根据肾小球滤过率值(GFR)将其分为早期肾损害组和肾功能正常组,分析各组间上述指标表达水平,并以GFR结果为金标准,计算血清Hcy、尿ACR和血清Cys-C检出结果和诊断效能。结果研究组Hcy、ACR、Cys-C均高于健康组(P<0.05),且随着高血压分级的增高,Hcy、ACR、Cys-C水平有升高趋势(P<0.05);83例原发性高血压患者中,发生早期肾损害39例(46.99%),肾功能正常44例(53.01%);早期肾损害组的Hcy、ACR、Cys-C水平均高于肾功能正常组(P<0.05);Hcy、ACR、Cys-C联合检测阳性率较高,其准确度、敏感度均高于单一指标检测(P<0.05)。结论Hcy、ACR联合Cys-C检测高血压早期肾损害患者能明显提高临床诊断效能,可作为今后对该类疾病的辅助检查手段。Objective To analyze the diagnostic value of homocysteine(Hcy),urinary microalbumin/creatinine ratio(ACR),and cystatin(Cys-C)in the diagnosis of early renal damage in hypertension.Method 83 patients with primary hypertension admitted to Jiayuguan Traditional Chinese Medicine Hospital from May 2021 to September 2023 were selected as the study group.According to the classification criteria for hypertension,they were divided into 24 cases at level 1,32 cases at level 2,and 27 cases at level 3.In addition,83 elderly patients who underwent health examinations during the same period were selected as the healthy group,and serum Hcy,urine ACR,and serum Cys-C were tested.The expression levels of each group’s indicators were compared;at the same time,83 patients with primary hypertension were counted for the occurrence of early renal damage.They were divided into early renal damage group and normal renal function group based on glomerular filtration rate(GFR).The expression levels of the above indicators were analyzed between each group,and the GFR results were used as the gold standard to calculate the detection results and diagnostic efficacy of serum Hcy,urine ACR,and serum Cys-C.Result The Hcy,ACR,and Cys-C levels in the study group were higher than those in the healthy group(P<0.05),and with the increase of hypertension grade,the levels of Hcy,ACR,and Cys-C showed an upward trend(P<0.05);among 83 patients with primary hypertension,39 cases(46.99%)experienced early renal damage and 44 cases(53.01%)had normal renal function;the levels of Hcy,ACR,and Cys-C in the early renal injury group were higher than those in the normal renal function group(P<0.05);the combined detection of Hcy,ACR,and Cys-C has a higher positive rate,with higher accuracy and sensitivity than single indicator detection(P<0.05).Conclusion The combination of Hcy,ACR,and Cys-C detection can significantly improve clinical diagnostic efficacy in patients with early renal damage in hypertension.It can be used as an auxiliary examination method for this t
