摘　　要：目的评估人工真皮治疗创伤创面临床疗效及探析人工真皮血管化时间的影响因素。方法采用前瞻性研究方法,纳入佛山市中医院修复重建外科2020年1月至2023年5月收治的创伤致全层皮肤缺损患者130例作为研究对象,评估人工真皮治疗的临床效果;采用单因素方差分析、多重线性回归分析人工真皮血管化的影响因素。结果130例患者创面均愈合良好,有效率100%,人工真皮血管化时间平均为(14.36±2.40)d。多重线性回归分析结果显示,人工真皮血管化时间的影响因素包括年龄、吸烟、糖尿病及术前Bates-Jensen伤口评估工具(BWAT)得分(P<0.05)。结论人工真皮治疗创伤创面疗效明确,年龄、吸烟、糖尿病及术前BWAT得分是人工真皮血管化时间的重要影响因素。在临床工作中,留意高龄患者,劝解患者戒烟、控制饮食、养成良好的生活习惯,对不同患者实施针对性措施以促进人工真皮修复患者创面快速愈合。Objective To evaluate the clinical efficacy of artificial dermis in treating traumatic wounds and to analyze the factors influencing the vascularization time of artificial dermis.Methods A prospective study was conducted on 130 patients with full-thickness skin defects due to trauma,treated at the Department of Repair and Reconstruction Surgery,Foshan Hospital of Traditional Chinese Medicine from January 2020 to May 2023.The clinical outcomes of artificial dermis treatment were assessed,and factors influencing dermal vascularization were analyzed using univariate ANOVA and multiple linear regression.Results All 130 patients had good wound healing,with an effective rate of 100%.The average time for artificial dermal vascularization was(14.36±2.40)days.Multiple linear regression analysis showed that age,smoking,diabetes,and preoperative Bates-Jensen Wound Assessment Tool(BWAT)scores were significant factors influencing the time of dermal vascularization(P<0.05).Conclusion Artificial dermis is effective in treating traumatic wounds.Age,smoking,diabetes,and preoperative BWAT scores are important factors influencing the vascularization time of artificial dermis.Clinicians should pay attention to elderly patients,encourage smoking cessation,control diet,and adopt targeted measures to promote rapid healing of wounds treated with artificial dermis.
