后路切开椎弓根钉棒系统内固定与经皮椎弓根钉棒系统复位内固定治疗单节段胸腰椎压缩性骨折的对比

摘　　要：目的:探讨将后路切开椎弓根钉棒系统内固定、经皮椎弓根钉棒系统复位内固定应用于单节段胸腰椎压缩性骨折患者中的作用与效果。方法:选取我院2018年3月—2021年3月收治的102例单节段胸腰椎压缩性骨折患者为研究对象,按照随机数字表法将其分为对照组与观察组,各51例。对照组采用后路切开椎弓根钉棒系统内固定,观察组采用经皮椎弓根钉棒系统复位内固定。比较2组患者的应用效果。结果:观察组治疗总有效率为98.04%,高于对照组的78.43%,差异有统计学意义(P<0.05)。术后,观察组疼痛视觉模拟评分低于对照组,失血总量、引流量均少于对照组,手术时间、住院时间均短于对照组,组间差异有统计学意义(P<0.05)。2组Cobb角、椎体前缘高度比较,组间差异无统计学意义(P>0.05)。结论:单节段胸腰椎压缩性骨折治疗工作中应用经皮椎弓根钉棒系统复位内固定较后路切开椎弓根钉棒系统内固定能够改善患者各项手术指标,并降低其VAS评分,能够获得更为理想的效果,促进其疾病康复,可推广。Objective:To investigate the effect of posterior incision pedicle screw system internal fixation and percutaneous pedicle screw system reduction internal fixation in patients with single level thoracolumbar compression fractures.Methods:102 patients with single-level thoracolumbar compression fracture admitted to our hospital from March 2018 to March 2021 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 51 cases in each group.The control group was treated with posterior incision pedicle screw system internal fixation,and the observation group was treated with percutaneous pedicle screw system reduction internal fixation.The application effect of the two groups was compared.Results:The total effective rate of the observation group was 98.04%,which was higher than 78.43%of the control group,the difference was statistically significant(P<0.05).After surgery,the Visual Analogue Score of pain in the observation group was lower than that in the control group,the total blood loss and drainage volume were lower than that in the control group,and the operation time and hospital stay were shorter than that in the control group,with statistical significance(P<0.05).There were no significant differences in Cobb Angle and vertebral anterior margin height between the two groups(P>0.05).Conclusion:In the treatment of single-level thoracolumbar compression fractures,the application of percutaneous pedicle screw system reduction and internal fixation can improve various surgical indicators of patients and reduce their VAS scores compared with posterior incision,which can achieve more ideal results and promote their disease rehabilitation,and can be popularized.
