不同入路腰椎椎体间融合术对腰椎椎管狭窄症的疗效分析

摘　　要：目的:分析不同入路腰椎椎体间融合术对腰椎椎管狭窄症的疗效。方法:回顾性分析,选取本院2018年3月—2020年3月收治的86例腰椎椎管狭窄症患者的临床资料,根据治疗方式的不同,将其分为研究组和对照组,各43例。对照组采用传统后路腰椎椎体间融合术治疗,研究组采用斜外侧入路腰椎椎体间融合术治疗,比较2组临床效果。结果:研究组手术时间长于对照组,术中出血量少于对照组,住院时间短于对照组,组间差异有统计学意义(P<0.05)。术后3d,研究组疼痛视觉模拟(VAS)评分、C反应蛋白及肌酸激酶均低于对照组,组间差异有统计学意义(P<0.05)。术后12个月,研究组Oswestry功能障碍指数评分为(17.52±0.64)分,低于对照组的(19.35±0.89),差异有统计学意义(P<0.05)。结论:斜外侧入路腰椎椎体间融合术用于治疗腰椎椎管狭窄患者,创伤应激较小,能够促进腰椎功能的恢复,缓解患者疼痛程度。Objective:To analyze the effect of lumbar interbody fusion with different approaches on lumbar spinal stenosis.Methods:The clinical data of 86 patients with lumbar spinal stenosis admitted to our hospital from March 2018 to March 2020 were retrospectively analyzed.According to different treatment methods,they were divided into a study group and a control group,with 43 cases.The control group was treated with traditional posterior lumbar interbody fusion,while the study group was treated with oblique lateral approach lumbar interbody fusion.The clinical effect of the two groups was compared.Results:The operation time of the study group was longer than that of the control group,the amount of intraoperative blood loss was less than that of the control group,and the length of hospital stay was shorter than that of the control group,the difference was statistically significant between the two group(P<0.05).3 days after surgery,the pain Visual Analogue Scale(VAS)score,C-reactive protein and creatine kinase in the study group were lower than those in the control group,and the differences between the two groups were statistically significant(P<0.05).At 12 months after surgery,the Oswestry disability index score of the study group was(17.52±0.64),which was lower than(19.35±0.89)of the control group,and the difference was statistically significant(P<0.05).Conclusion:Oblique lateral c lumbar interbody fusion for the treatment of lumbar spinal stenosis patients,less traumatic stress,can promote the recovery of lumbar function,relieve the pain degree of patients.
