右胫骨高位截骨内固定术联合左侧髂骨取骨术在膝关节骨性关节炎患者治疗中的应用效果

摘　　要：目的:探讨右胫骨高位截骨内固定术联合左侧髂骨取骨术在膝关节骨性关节炎患者治疗中的应用效果.方法:选取2019年4月-2021年4月山东省文登整骨医院收治的60例膝关节骨性关节炎患者为研究对象,采用随机数字表发分为对照组与观察组,各30例.对照组采用腓骨截骨术治疗,观察组采用右胫骨高位截骨内固定术联合左侧髂骨取骨术治疗,比较2组手术指标与治疗效果.结果:观察组术中出血量、手术时间、住院时间、骨折愈合时间均低于对照组,差异有统计学意义(P<0.05).治疗前,观察组视觉模拟评分表(VAS)、西方人膝关节和髋关节疼痛和残疾问卷(WOMAC)、膝关节功能评估量表(Lysholm)以及髌股关节的骨骼轴位指标(HKA)角角度与对照组比较,差异无统计学意义(P>0.05);治疗后,观察组VAS评分、WOMAC评分低于对照组,Lysholm评分高于对照组,HKA角角度大于对照组,差异有统计学意义(P<0.05).结论:右胫骨高位截骨内固定术联合左侧髂骨取骨术在膝关节骨性关节炎患者治疗中的应用效果较好.Objective:To explore the application effect of high tibial osteotomy with internal fixation of the right tibia combined with iliac bone grafting on the left side in patients with knee osteoarthritis.Methods:Sixty patients with knee osteoarthritis treated at Wendeng Orthopedic Hospital of Shandong Province from April 2019 to April 2021 were selected as the research subjects and randomly divided into control group and observation group,with 30 cases in each group,using a random number table.The control group was treated with fibular osteotomy,and the observation group was treated with high tibial osteotomy with internal fixation of the right tibia combined with iliac bone grafting on the left side.The surgical indicators and treatment effects of the two groups were compared.Results:The intraoperative blood loss,operation time,length of hospital stay,and fracture healing time in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Before treatment,there was no statistically significant difference in Visual Analog Scale(VAS),Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC),Lysholm Knee Scoring Scale,and Hip-Knee-Ankle(HKA)angle between the two groups(P>0.05);after treatment,the VAS score and WOMAC score in the observation group were lower than those in the control group,the Lysholm score was higher than that in the control group,and the HKA angle was greater than that in the control group,and the differences were statistically significant(P<0.05).Conclusion:The application effect of high tibial osteotomy with internal fixation of the right tibia combined with iliac bone grafting on the left side is better in the treatment of patients with knee osteoarthritis.
