微创经皮锁定钢板接骨术对老年桡骨远端骨折患者掌倾角及腕关节功能的影响

摘　　要：目的:探讨微创经皮锁定钢板接骨术对老年桡骨远端骨折患者掌倾角及腕关节功能的影响。方法:选取2019年1月-2023年1月我院收治的60例老年桡骨远端骨折患者为研究对象,随机将其分为对照组和观察组,各30例。对照组采用外固定手术疗法与手法复位治疗,观察组采用微创经皮锁定钢板接骨术治疗。对比2组的临床疗效、桡骨高度、尺偏角、掌倾角、腕关节功能、并发症发生率。结果:观察组优良率高于对照组,差异有统计学意义(P<0.05)。术后6个月,观察组桡骨高度高于对照组,尺偏角、掌倾角均大于对照组,组间差异有统计学意义(P<0.05)。术后6个月,2组患者疼痛、功能状况、活动范围、握力评分均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率比对照组更低,差异有统计学意义(P<0.05)。结论:与常规外固定手术相比,微创经皮锁定钢板接骨术治疗老年桡骨远端骨折的疗效更加突出,可改善腕关节平衡度和掌倾角,有利于患者腕关节功能的快速恢复,同时还具有较好的安全性。Objective:To investigate the effect of minimally invasive percutaneous locking plate osteosynthesis on palmar inclination and wrist function in elderly patients with distal radius fractures.Methods:60 elderly patients with distal radius fractures admitted to our hospital from January 2019 to January 2023 were selected as the research subjects,randomly divided them into a control group and an observation group,with 30 cases in each group.The control group received external fixation surgery and manual reduction treatment,while the observation group received minimally invasive percutaneous locking plate osteosynthesis treatment.The clinical efficacy,radius height,ulnar deviation angle,palm angle,wrist joint function,and incidence of complications between two groups were compared.Results:The excellent rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).After 6 months of surgery,the height of the radius in the observation group was higher than that in the control group,and the ulnar deviation angle and palmar inclination angle were higher than those in the control group,the difference was statistically significant(P<0.05).Six months after surgery,the pain,functional status,range of motion,and grip strength scores of both groups of patients were higher than before surgery,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Compared with conventional external fixation surgery,minimally invasive percutaneous locking plate osteosynthesis has a more prominent therapeutic effect in the treatment of distal radius fractures in the elderly.It can improve wrist balance and palmar inclination,facilitate rapid recovery of wrist function in patients,and also have good safety.
