摘　　要：目的探究幽门螺杆菌(Hp)与颈动脉粥样硬化狭窄程度及血清脂蛋白相关磷脂酶A2(Lp-PLA2)、基质金属蛋白酶-9(MMP-9)、内脏脂肪素(visfatin)的关系。方法选取河南大学准河医院2021年7月-2023年7月收治的89例颈动脉粥样硬化患者为研究对象,根据是否发生Hp感染分为感染组28例和非感染组61例;另根据颈动脉超声结果,分为轻、中和重度狭窄组分别为25、31和33例。比较感染组与非感染组及不同程度狭窄组之间血清Lp-PLA2、MMP-9、visfatin水平。采用Spearman分析Hp感染与颈动脉粥样硬化患者超声评估狭窄程度及血清Lp-PLA2、MMP-9、visfatin水平的相关性。结果中、重度狭窄组血清Lp-PLA2、MMP-9、visfatin水平高于轻度狭窄组,且重度狭窄组高于中度狭窄组(P<0.05);重度狭窄组Hp感染阳性患者占比高于轻度狭窄组(P<0.05)。Hp感染与颈动脉粥样硬化患者超声评估狭窄程度呈正相关(P<0.05)。感染组血清Lp-PLA2、MMP-9、visfatin水平均高于非感染组(P<0.05)。Hp感染与颈动脉粥样硬化患者血清Lp-PLA2、MMP-9、visfa-tin水平呈正相关(P<0.05)。结论仑颈动脉粥样硬化患者Hp感染的血清Lp-PLA2、MMP-9、visfatin水平呈高表达;且Hp感染与颈动脉粥样硬化患者狭窄程度及血清Lp-PLA2、MMP-9、visfatin水平有相关性。OBJECTIVE To explore the relationship between Helicobacter pylori and degree of carotid atherosclero-sis stenosis,serum lipoprotein-associated phospholipase A2(Lp-PLA2),matrix metalloproteinase-9(MMP-9)and visfatin.METHODS A total of 89 patients with carotid atherosclerosis who were treated in Huaihe Hospital of Henen University from Jul 2021 to Jul 2023 were recruited as the research subjects and were divided into the infec-tion group with 28 cases and the non-infection group with 6l cases according to the status of Hp infection.The pa-tients were divided into the mild stenosis group with 25 cases,the moderate stenosis group with 31 cases and the severe stenosis group according to the result of carotid artery ultrasound examination.The levels of serum Lp-PLA2,MMP-9 and visfatin were compared between the infection group and the non-infection group as well as a-mong the patients with different severities of stenosis.Spearman analysis was performed for the association be-tween the Hp infection and the severity of carotid atherosclerosis stenosis,serum Lp-PLA2,MMP-9 and visfatin.RESULTS The levels of serum Lp-PLA2,MMP-9 and visfatin of the moderate stenosis group and the severe ste-nosis group were significantly higher than those of the mild stenosis group,and the levels of above indexes of the severe stenosis group were significantly higher than those of the moderate stenosis group(P<0.05).The percentage of Hp-positive patients of the severe stenosis group was significantly higher than that of the mild stenosis group(P<0.05).The Hp infection was positively correlated with the severity of stenosis assessed by the ultrasound examination(P<0.05).The levels of serum Lp-PLA2,MMP-9 and visfatin of the infection group were significantly higher than those of the non-infection group(P<o.o5).The Hp infection was positively correlated with the levels of serum Lp-PLA2,MMP-9 and visfatin of the patients with carotid atherosclerosis(P<o.05).CONCLUSION The carotid atherosclerosis patients with Hp infection show high expression le
