摘　　要：目的 探讨Sextant微创经皮椎弓根螺钉内固定术治疗脊柱骨折的效果,及对患者临床指标和脊柱功能的影响。方法 选取2022年6月—2023年6月锦州医科大学第一临床医学院收治的82例胸腰椎段脊柱骨折患者为研究对象,以双盲法分为两组,各41例。开放组应用开放式椎弓根螺钉内固定术,微创组应用Sextant微创经皮椎弓根螺钉内固定术。比较两组临床疗效。结果 两组手术时间对比,差异无统计学意义(P>0.05);微创组出血量、切口长度、住院时间均优于开放组,差异有统计学意义(P均<0.05);手术后,微创组椎体矢状面指数、椎体前缘高度均明显大于开放组,Cobb角明显小于开放组,差异有统计学意义(P均<0.05);手术后,微创组脊柱功障碍能评分和疼痛评分分别为(14.03±3.24)分、(2.63±0.87)分,低于对照组的(19.43±4.75)分、(5.32±1.16)分,差异有统计学意义(t=6.014、11.879,P均<0.05);微创组并发症发生率低于开放组,差异有统计学意义(P<0.05)。结论 Sextant微创经皮椎弓根螺钉内固定术疗效确切,对患者造成的创伤小,有利于患者椎旁肌肉和韧带复合体稳定,保护腰背功能,同时该术式术后并发症少,可为患者术后脊柱功能以及术后恢复提供有利条件。Objective To investigate the effect of Sextant minimally invasive percutaneous pedicle screw fixation on spinal fracture and its effect on clinical indexes and spinal function.Methods A total of 82 patients with thoracolum-bar spine fracture admitted to the First Clinical Medical School of Jinzhou Medical University from June 2022 to June 2023 were selected as the study objects and were divided into two groups with 41 cases each by double-blind method.Open pedicle screw internal fixation was used in the open group,and Sextant minimally invasive percutaneous pedicle screw internal fixation was used in the minimally invasive group.The clinical efficacy of the two groups was compared.Results There was no statistically significant difference in operation time between the two groups(P>0.05).The amount of blood loss,incision length and hospital stay in the minimally invasive group were better than those in the open group,and the differences were statistically significant(all P<0.05).After surgery,the sagittal plane index and anterior margin height of vertebrae in the minimally invasive group were significantly higher than those in the open group,and the Cobb's angle was significantly smaller than that in the open group,and the differences were statisti-cally significant(all P<0.05).After surgery,the scores of spinal dysfunction scores and pain scores in the minimally invasive group were 14.03±3.24 and 2.63±0.87,respectively,lower than 19.43±4.75 and 5.32±1.16 in the open group,and the differences were statistically significant(t=6.014,11.879,both P<0.05).The incidence of complications in minimally invasive group was lower than that in open group,and the difference was statistically significant(P<0.05).Conclusion Sextant minimally invasive percutaneous pedicle screw internal fixation is effective and causes little trauma to the patient,which is conducive to the stability of the paravertebral muscle and ligament complex of the pa-tient and the protection of lumbar and back function.At the same time,this operation has
