摘　　要：结核病是由结核分枝杆菌引起的全球最致命的慢性疾病之一。作为一种主要寄生于巨噬细胞的兼性胞内菌,结核分枝杆菌通过对巨噬细胞的免疫逃避和免疫抑制使其能够在宿主体内保持潜伏状态,难以杀灭。因此,杀灭和抑制巨噬细胞内结核分枝杆菌对于确保结核病的治愈十分重要。传统抗结核药物治疗存在生物利用度低、用药周期长、毒性大等缺点。主动靶向巨噬细胞的抗结核药物递送系统可通过与巨噬细胞产生配体-受体相互作用来增加巨噬细胞对载药系统的吞噬,从而在巨噬细胞内达到有效的杀菌浓度。笔者将以结核分枝杆菌的致病机制及其对巨噬细胞的免疫逃避和免疫抑制为背景,对主动靶向巨噬细胞抗结核药物递送系统的研究进展进行综述。Tuberculosis is one of the deadliest chronic diseases worldwide caused by Mycobacterium tuberculosis.As a facultative intracellular bacterium that mainly parasitizes macrophages,Mycobacterium tuberculosis is difficult to kill by maintaining a latent state in the host through immune evasion and immunosuppression of macrophages.Therefore,killing and inhibiting Mycobacterium tuberculosis in macrophages is important to ensure the cure of tuberculosis.Traditional anti-tuberculosis drug therapy has the disadvantages of low bioavailability,long medication cycle,and high toxicity.Antituberculous drug delivery systems that actively target macrophages can increase phagocytosis of drug-loaded systems by macrophages by producing ligand-receptor interactions with macrophages,thereby achieving effective bactericidal concentrations within macrophages.The authors will review the research progress of active targeting of anti-tuberculosis drug delivery systems in macrophages against the background of the pathogenic mechanism of Mycobacterium tuberculosis and its immune evasion and immunosuppression of macrophages.
