摘　　要：目的探讨加速康复外科(ERAS)理念在儿童腹股沟疝围手术期中的应用。方法选择2021年1—10月,就诊于连云港市第一人民医院,行腹腔镜疝囊高位结扎手术的110例腹股沟疝患儿作为研究对象,采用随机数字表法分为A组和B组,各55例。A组采用常规儿童腹股沟疝围手术期护理模式,B组在A组基础上采用ERAS理念护理模式。对比2组患儿围手术期指标和干预前后疼痛反应程度、治疗依从性、心理健康状况及家长护理满意度、并发症发生情况。结果B组患儿肠鸣音恢复时间、首次进食时间、下床时间和住院时间少于A组(P<0.05);B组FLACC疼痛程度和少儿心理健康量表(MHS-CA)评分低于A组,Frankl治疗依从性分级量表(FCS)评分高于A组(P<0.05);B组患儿家长护理满意度各项评分明显高于A组(P<0.05);B组患儿术后并发症总发生率(10.90%)低于A组(25.46%),差异无统计学意义(P>0.05)。结论ERAS理念护理干预可有效缓解腹股沟疝手术患儿围手术期疼痛反应程度,改善治疗依从性及心理健康状况,提高家长护理满意度。Objective To explore the application of the enhanced recovery after surgery(ERAS)concept in the perioperative period of pediatric inguinal hernia.Methods A total of 110 children with inguinal hernia undergoing laparoscopic high ligation of the hernia sac in Lianyungang First People's Hospital were enrolled as research subjects between January and October 2021.According to the random number table method,they were divided into groups A(55 cases)and B(55 cases).Group A was given routine perioperative nursing,while group B was additionally given ERAS concept nursing.The perioperative indexes,pain response,treatment compliance,mental health status,parental nursing satisfaction and occurrence of complications were compared between the two groups before and after intervention.Results The recovery time of bowel sound,the first eating time,leaving bed time,and hospitalization time in group B were shorter than those in group A(P<0.05).The scores of the FLACC pain degree and mental health scale for children and adolescents(MHS-CA)in group B were lower than those in group A,and the score of the Frankl compliance scale(FCS)was higher than that in group A(P<0.05).The scores of parental nursing satisfaction in group B were significantly higher than those in group A(P<0.05).The total incidence of postoperative complications in group B was lower than that in group A(10.90%vs 25.46%),but the difference was not statistically significant between the two groups(P>0.05).Conclusion The nursing intervention based on the ERAS concept can effectively alleviate perioperative pain,improve treatment compliance,mental health status and parental nursing satisfaction in children with inguinal hernia.
