摘　　要：目的了解MSM HIV自我检测现况及相关因素,为扩大MSM HIV自我检测提供科学依据。方法采用横断面调查方法,于2022年4月至2023年10月,采用滚雪球抽样方法招募MSM开展问卷调查,调查内容包括一般人口学资料、性行为特征、HIV检测行为特征等内容。采用多因素Logistics回归模型对最近一年HIV自我检测的相关因素进行分析。结果共招募806名MSM,平均年龄(35.1±10.1)岁,25~39岁占51.2%,本省户籍占62.0%,最近3个月通过互联网寻找性伴占90.3%。最近一年MSM做过HIV自我检测比例为42.7%(344/806)。Logistic回归模型多因素分析显示大专及以上(aOR=2.21,95%CI:1.41~3.48),性角色为“仅插入方”(aOR=1.97,95%CI:1.30~2.99),HIV相关知识全部知晓(aOR=2.29,95%CI:1.65~3.18),最近3个月通过互联网交友(aOR=2.84,95%CI:1.40~5.75),既往在医疗机构做过HIV检测(aOR=2.04,95%CI:1.46~2.84)的MSM最近一年进行HIV自我检测的比例更高。结论浙江省MSM HIV自我检测比例为42.7%,建议探索利用互联网平台尤其是MSM交友软件开展HIV相关知识的宣传教育,重点对低文化水平MSM开展HIV检测知识的宣传教育,倡导促进MSM高危行为后进行HIV自我检测。Objective To understand the current status of self-testing for HIV among men who have sex with men(MSM)and its associated factors,in order to provide scientific evidence for expanding HIV self-testing among MSM.Methods A cross-sectional survey was conducted from April 2022 to October 2023.MSM were recruited using a snowball sampling method,and a questionnaire survey was conducted,which included general demographic information,sexual behavior characteristics,and HIV testing behavior characteristics.Multivariable logistic regression models were used to analyze the factors associated with recent self-testing for HIV among MSM.Results A total of 806 MSM with a mean age of(35.1±10.1)years were recruited.The age group of 25-39 years accounted for 51.2%,and those with local household registration accounted for 62.0%.In the last three months,90.3%of MSM had sought sexual partners through the Internet.The proportion of MSM who had performed self-testing for HIV in the last year was 42.7%(344/806).Multivariable logistic regression model revealed that MSM with a college degree or above(aOR=2.21,95%CI:1.41-3.48),those who identified as"insertive only"in their sexual role(aOR=1.97,95%CI:1.30-2.99),those with thorough awareness of knowledge HIV(aOR=2.29,95%CI:1.65-3.18),those who had used the internet for dating in the past three months(aOR=2.84,95%CI:1.40-5.75),and those who had previously been tested for HIV in medical institutions(aOR=2.04,95%CI:1.46-2.84)were more likely to have performed self-testing for HIV in the last year.Conclusions The proportion of MSM self-testing for HIV in Zhejiang Province is relatively low.It is recommended to explore the use of internet platforms,especially MSM dating APPs,for HIV-related knowledge promotion and education.Special attention should be given to MSM with lower education levels in HIV testing knowledge promotion and education.Self-testing for HIV among MSM after engaging in high-risk behaviors should be advocated.
