桃核承气汤治疗血管性痴呆(阳明蓄血证)的疗效及对神经电生理的影响

摘　　要：目的分析桃核承气汤治疗血管性痴呆(vascular dementia,VD)的疗效及对神经电生理的影响。方法选取我院2021年3月至2023年5月收治的94例VD患者为研究对象,依据随机数字表法1∶1分为对照组、观察组,各47例。对照组采用常规西药治疗,观察组在其基础上增加桃核承气汤治疗。比较两组临床疗效、治疗前后简易智能精神状态量表(MMSE)、VD辨证量表(SDSVD)、Barthel指数(BI)、临床痴呆程度量表(CDR)、中医证候积分、神经电生理(P300潜伏期、P300振动)、大脑中动脉(MCA)及大脑前动脉(ACA)血流速水平。结果观察组临床疗效95.74%高于对照组80.85%,差异有统计学意义(P<0.05);治疗后观察组Barthel指数、MMSE评分高于对照组,SDSVD、CDR评分低于对照组,差异有统计学意义(P<0.05);治疗后两组记忆减退、言辞颠倒及神情呆钝积分降低,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后两组性情改变积分比较无明显差异(P>0.05);治疗后观察组P300潜伏期水平低于对照组,P300振动、ACA、MCA水平高于对照组,差异有统计学意义(P<0.05)。结论桃核承气汤治疗VD能加快脑血流速度,改善临床症状及认知、记忆等功能,疗效确切。Objective To analyze the effect of Taohe Chengqi Decoction on vascular dementia(VD)and its effect on neuroelectrophysiology.Methods Ninety-four cases of VD patients in our hospital from March 2021 to May 2023 were selected as the research object and randomly divided into the control group and the observation group,with forty-seven cases in each group.The control group was treated with conventional Western medicine,and the observation group was treated with Taohe Chengqi decoction.The clinical efficacy,simple Intelligent mental State Scale(MMSE),VD syndrome Differentiation Scale(SDSVD),Barthel index(BI),Clinical dementia Degree Scale(CDR),TCM syndrome score,neuroelectrophysiology(P300 latency,P300 vibration),middle cerebral artery(MCA)and anterior cerebral artery(ACA)Blood flow rate level were compared between the two groups.Results The clinical effect of the observation group was 95.74%higher than that of the control group 80.85%,and the difference was statistically significant(P<0.05).After treatment,Barthel index and MMSE scores in the observation group were higher than those in the control group,and SDSVD and CDR scores were lower than those in the control group,with statistical significance(P<0.05).After treatment,the scores of memory loss,speech reversal,and expression dullness were decreased in two groups,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).There was no significant difference in temperament change scores between the two groups after treatment(P>0.05).After treatment,the P300 latency level in the observation group was lower than in the control group,and P300 vibration,ACA,and MCA levels were higher than the control group,the difference was statistically significant(P<0.05).Conclusion Taohe Chengqi Decoction can accelerate cerebral blood flow rate,and improve clinical symptoms,cognition,memory and other functions in the treatment of VD.
