心脏彩超诊断冠心病及评估患者室壁运动异常的效果分析

摘　　要：目的探讨心脏彩超对冠心病(CHD)患者的诊断与室壁运动异常的评估效果分析。方法选取2021年3月至2023年6月郑州市金水区总医院收治的100例疑似冠心病患者,以冠状动脉造影为“金标准”分为冠心病组(n=80)和非冠心病组(n=20),均接受心脏彩超检查,比较冠心病组和非冠心病组心脏彩超指标(左心室射血分数、左心室收缩末期容积、左心室舒张末期容积、每博量);并采用灵敏度、特异度、约登指数分析心脏彩超的诊断效能及心脏彩超诊断冠心病节段性室壁运动异常的效果。结果冠心病组患者左心室收缩末期容积高于非冠心病组,左心室射血分数、左心室舒张末期容积、每博量低于非冠心病组,差异有统计学意义(P<0.05);以冠状动脉造影检查为金标准,心脏彩超的准确度、灵敏度、特异度、约登指数分别为97.00%、97.50%、95.00%、0.815;冠状动脉造影检查显示80例患者存在不同程度节段性室壁运动异常,心脏彩超检查显示75例患者存在节段性室壁运动异常,检出率为93.75%。结论心脏彩超检查对诊断冠心病患者具有一定价值,可使患者的室壁运动异常得到有效评估。Objective To explore the effectiveness of cardiac ultrasound in diagnosing coronary heart disease(CHD)and evaluating wall motion abnormalities in patients.Methods One hundred patients with suspected CHD treated at Jinshui District General Hospital from March 2021 to June 2023 were selected.Using coronary angiography as the“gold standard,”patients were divided into the CHD group(n=80)and the non-CHD group(n=20).All patients underwent cardiac ultrasound examination.Cardiac ultrasound indicators[left ventricular ejection fraction(LVEF),left ventricular end-systolic volume(LVESV),left ventricular end-diastolic volume(LVEDV),stroke volume(SV)]were compared between the CHD group and the non-CHD group.Sensitivity,specificity,and Youden index were used to analyze the diagnostic efficacy of cardiac ultrasound and its effectiveness in diagnosing segmental wall motion abnormalities in CHD patients.Results The LVESV in the CHD group was higher than that in the non-CHD group,while the LVEF,LVEDV,and SV were lower than those in the non-CHD group,with statistically significant differences(P<0.05).Using coronary angiography as the gold standard,the accuracy,sensitivity,specificity,and Youden index of cardiac ultrasound were 97.00%,97.50%,95.00%,and 0.815,respectively.Coronary angiography showed that 80 patients had varying degrees of segmental wall motion abnormalities,while cardiac ultrasound detected segmental wall motion abnormalities in 75 patients,with a detection rate of 93.75%.Conclusion Cardiac ultrasound examination has certain value in diagnosing CHD patients and can effectively evaluate wall motion abnormalities.
