高频彩色多普勒超声血流成像诊断乳腺恶性肿块的临床应用分析

摘　　要：目的探究乳腺恶性肿块的临床诊断应用高频彩色多普勒超声血流成像的价值。方法研究样本均为平舆县中医院超声科于2022年2月至2023年12月期间收入72例乳腺肿块患者,对所有样本对象均实施高频彩色多普勒超声血流成像,并以手术病理诊断为金标准,分析其在诊断乳腺肿块良恶性的临床应用价值。结果72例研究样本中,手术病理诊断为良性肿块45例,占比情况为62.50%,恶性肿块27例,占比情况为37.50%,高频彩色多普勒超声血流成像检测诊断结果为真阳性25例,假阳性1例,真阴性44例,假阴性2例,其敏感度为92.59%(25/27)、准确度为95.83%(69/72)、特异度为97.78%(44/45),其与手术病理诊断具有较高一致性(Kappa=0.746);良性与恶性肿块的血流成像特征情况方面,良性肿块形态、边缘、边界、微钙化灶情况均与恶性肿块比较,差异有统计学意义(P<0.05);良性肿块血管最高搏动指数、阻力指数、动脉血流峰值、收缩期峰值流速均低于恶性肿块,差异有统计学意义(P<0.05)。结论乳腺恶性肿块的临床诊断中,应用高频彩色多普勒超声血流成像的价值较为理想,能够准确判断其良恶性情况,益于早期治疗,具有临床应用意义。Objective To explore the value of high-frequency color Doppler ultrasound blood flow imaging in the clinical diagnosis of malignant breast masses.Methods The research samples were 72 patients with breast masses admitted to the Ultrasound Department of Pingyu County Traditional Chinese Medicine Hospital from February 2022 to December 2023.All sample subjects underwent high-frequency color Doppler ultrasound blood flow imaging,and surgical pathology diagnosis was used as the gold standard to analyze its clinical application value in diagnosing the benign and malignant nature of breast masses.Results Among the 72 research samples,45 cases were diagnosed as benign masses by surgical pathology,accounting for 62.50%,and 27 cases were malignant masses,accounting for 37.50%.The results of high-frequency color Doppler ultrasound blood flow imaging detection and diagnosis were 25 true positives,1 false positive,44 true negatives,and 2 false negatives.Its sensitivity was 92.59%(25/27),accuracy was 95.83%(69/72),specificity was 97.78%(44/45),and it had a high consistency with surgical pathology diagnosis(Kappa=0.746).In terms of the blood flow imaging characteristics of benign and malignant masses,the morphology,edge,boundary,and microcalcification of benign masses were all compared with malignant masses,and the difference was statistically significant(P<0.05).The maximum pulsatility index,resistance index,peak arterial blood flow,and peak systolic velocity of the blood vessels of benign masses were all lower than those of malignant masses,and the difference was statistically significant(P<0.05).Conclusion In the clinical diagnosis of malignant breast masses,the application of highfrequency color Doppler ultrasound blood flow imaging is ideal,which can accurately judge its benign and malignant conditions,benefit early treatment,and has clinical application significance.
