摘　　要：目的探讨采用柔性管理法对胫腓骨开放性骨折钢板固定术后患者负压引流(VSD)治疗依从性及应对方式的影响。方法将南阳张仲景医院自2020年3月至2022年11月期间创伤骨科收治的70例胫腓骨开放性骨折患者列为研究对象。按照密闭信封随机抽签法分为对照组(n=35)与实验组(n=35),对照组采用术后传统护理,实验组采用柔性管理法,比较两组患者干预后护理依从性及干预前后下肢功能评分(LEFS)、自我应对能力[特质应对方式评分(TCSQ)]和护理期间术后并发症发生率,并统计后期随访过程中两组患者生活质量[生活质量评分精简版(SF-12)]。结果干预后,实验组患者护理依从性、LEFS评分、SF-12评分均高于对照组,差异具有统计学意义(P<0.05);干预后,两组患者TCSQ应对能力中,实验组积极应对评分(PC)高于对照组,消极应对评分(NC)低于对照组,差异有统计学意义(P<0.05);实验组术后并发症发生率低于对照组,差异具有统计学意义(P<0.05)。结论采用柔性管理法可显著提升胫腓骨开放性骨折钢板固定术后患者依从性,在面对自身疾病状况时能采取积极有效的应对方式,减少并发症发生风险,恢复患者下肢功能,提高患者生活质量。Objective To explore the impact of flexible management on the compliance and coping strategies of patients with Vacuum Sealing Drainage(VSD)treatment after open tibiofibular fracture plate fixation surgery.Methods 70 patients with open tibiofibular fractures admitted to the Department of Trauma Orthopedics of Nanyang Zhang Zhongjing Hospital from March 2020 to November 2022 were selected as the research subjects.They were divided into a control group(n=35)and an experimental group(n=35)according to the method of sealed envelope random draw.The control group was given postoperative traditional nursing,and the experimental group was given flexible management.The nursing compliance,lower extremity function score(LEFS),self-coping ability[trait coping style questionnaire(TCSQ)],and the incidence of postoperative complications during nursing were compared between the two groups.The quality of life SF-12 of the two groups of patients was also statistically analyzed during the follow-up period.Results After intervention,the nursing compliance,LEFS score,and SF-12 score of the experimental group were all higher than those of the control group,and the differences were statistically significant(P<0.05).After intervention,in the TCSQ coping ability of the two groups of patients,the positive coping score(PC)of the experimental group was higher than that of the control group,and the negative coping score(NC)was lower than that of the control group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The use of flexible management can significantly improve the compliance of patients after open tibiofibular fracture plate fixation surgery,adopt positive and effective coping strategies when facing their own disease conditions,reduce the risk of complications,restore the function of the patient’s lower limbs,and improve the quality of life of pa
