Endo-ULBD治疗老年腰椎管狭窄症的临床效果

摘　　要：目的:探究内镜下单侧椎板间入路双侧椎管减压(endoscopy-unilateral laminectomy for bilateral decompression,Endo-ULBD)治疗老年腰椎管狭窄症的临床效果。方法:选取2023年2—11月思南县人民医院脊柱外科收治的64例老年腰椎管狭窄症患者。根据Excel随机数发生器分组法将其分为参照组和研究组,各32例。参照组接受开放减压融合内固定术,研究组接受Endo-ULBD。比较两组围手术期指标,手术效果,术前、术后3个月腰椎功能及并发症。结果:研究组手术时间、住院时间均短于对照组,术中出血量少于对照组,医疗费用高于对照组,差异有统计学意义(P<0.05)。术后3个月,研究组优良率高于参照组,差异有统计学意义(P<0.05)。术后3个月,研究组Oswestry功能障碍指数(Oswestry disability index,ODI)评分低于参照组,差异有统计学意义(P<0.05)。研究组并发症发生率低于参照组,差异有统计学意义(P<0.05)。结论:Endo-ULBD治疗老年腰椎管狭窄症优于开放减压融合内固定术,能够减轻手术创伤,提升临床疗效,减少并发症,但会增加医疗费用。Objective:To explore the clinical effect of endoscopy-unilateral laminectomy for bilateral decompression(Endo-ULBD)in the treatment of senile lumbar spinal stenosis.Method:A total of 64 elderly patients with lumbar spinal stenosis admitted to the Spinal Surgery Department of Sinan County People's Hospital from February to November 2023 were selected.According to Excel random number generator grouping method,they were divided into reference group and study group,32 cases in each group.The control group received the open decompression fusion and internal fixation surgery,the study group underwent Endo-ULBD.The perioperative indexes,operative effect,lumbar function before and 3 months after surgery and complications were compared between two groups.Result:The operation time and hospital stay of the study group were shorter than those of the control group,the amount of intraoperative blood loss was less than that of the control group,and the medical cost was higher than that in control group,the differences were statistically significant(P<0.05).Three months after surgery,the excellent and good rate of the study group was higher than that of the reference group,and the difference was statistically significant(P<0.05).At 3 months after surgery,the Oswestry disability index(ODI)score of the study group was lower than that of the reference group,and the difference was statistically significant(P<0.05).The incidence of complication of the study group was lower than that of the reference group,and the difference was statistically significant(P<0.05).Conclusion:Endo-ULBD is superior to open decompression fusion and internal fixation surgery in the treatment of senile lumbar spinal stenosis,which can reduce surgical trauma,improve clinical efficacy and reduce complications,but it will increase medical expenses.
