新安解燥汤加减联合中药浸泡治疗慢性手部湿疹临床研究

摘　　要：目的观察新安解燥汤加减联合中药浸泡治疗慢性手部湿疹的临床疗效。方法将60例慢性手部湿疹患者随机分为对照组和观察组,每组30例。对照组采用慢性手部湿疹外洗方浸泡后立即外用尿素乳膏,观察组在对照组基础上予以新安解燥汤加减治疗。分别比较治疗前、治疗4周后、治疗8周后两组患者手部湿疹严重指数(hand eczema severity index,HECSI)评分、瘙痒和疼痛的视觉模拟量表(visual analogue scale,VAS)评分以及皮肤病生活质量指数(dermatology life quality index,DLQI)评分,并评估临床疗效、不良反应及复发情况。结果两组患者治疗4周后HECSI评分、VAS评分、DLQI评分均显著低于治疗前(P<0.05),治疗8周后上述评分均显著低于治疗4周后(P<0.05),且观察组降低程度更明显(P<0.05)。观察组临床疗效优于对照组(P<0.05),且复发率更低(P<0.05),两组均无明显不良反应。结论新安解燥汤加减联合中药浸泡治疗慢性手部湿疹临床疗效确切。Objective To investigate the clinical effect of modified Xin an Jiezao Decoction combined with Chinese herbal immersion in the treatment of chronic hand eczema.Methods A total of 60 patients with chronic hand eczema were randomly divided into control group and observation group,with 30 patients in each group.The patients in the control group received soaking with prescribed external wash for chronic hand eczema,followed by external application of urea cream,and those in the observation group received modified Xin an Jiezao Decoction in addition to the treatment in the control group.The two groups were compared in terms of Hand Eczema Severity Index(HECSI),Visual Analogue Scale(VAS)scores of itching and pain,and Dermatology Life Quality Index(DLQI)before treatment and after 4 and 8 weeks of treatment,and overall therapeutic efficacy,adverse reactions,and recurrence were also evaluated.Results Both groups had significant reductions in HECSI,VAS,and DLQI scores after 4 weeks of treatment(P<0.05),and these scores after 8 weeks of treatment were significantly lower than those after 4 weeks of treatment(P<0.05),with significantly greater reductions in the observation group(P<0.05).Compared with the control group,the observation group had a significantly better treatment outcome(P<0.05)and a significantly lower recurrence rate(P<0.05),with little adverse effect in either group.Conclusion Modified Xin an Jiezao Decoction combined with Chinese herbal immersion has a marked clinical effect in the treatment of chronic hand eczema.
