调脾安肠方联合虎符铜砭刮痧治疗老年糖尿病胃轻瘫所致便秘临床疗效

摘　　要：目的:观察调脾安肠方联合虎符铜砭刮痧治疗老年糖尿病胃轻瘫(DGP)所致便秘的疗效。方法:将82例老年DGP所致便秘患者随机分为两组,对照组(n=41)给予西医治疗,观察组(n=41)在其基础上给予调脾安肠方联合虎符铜砭刮痧治疗,疗程均为4周。比较两组疗效,记录两组治疗前后胃轻瘫症状严重指数(GCSI)、便秘患者症状自评量表(PAC-SYM)、胃排空时间和胃排空率、胃电图参数、血糖和生活质量的变化。记录两组不良反应和复发率。结果:观察组治疗总有效率高于对照组(P<0.05)。与治疗前比较,两组治疗后GCSI评分、PAC-SYM评分、胃电节律紊乱百分比、血糖指标(空腹血糖、餐后2 h血糖和糖化血红蛋白)、便秘患者生活质量量表(PAC-QOL)各维度评分和总分明显降低,胃排空率、平均收缩波频率(MFC)、平均收缩波振幅(AC)显著增加(均P<0.05),且观察组治疗后的上述指标优于对照组(均P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。观察组复发率低于对照组(P<0.05)。结论:调脾安肠方联合虎符铜砭刮痧治疗能提高老年DGP所致便秘的临床疗效,不仅能缓解患者症状,改善胃排空能力和胃肠动力,还能调节血糖,改善便秘患者生活质量,降低复发率,且安全性好。Objective:To observe the clinical efficacy of Tiaopi Anchang recipe combined with Hufu Tongbian Guasha in the treatment of constipation caused by diabetic gastroparesis(DGP)in the elderly.Methods:A total of 82 elderly patients with constipation caused by DGP were randomly divided into two groups,the control group was treated with western medicine,and the observation group was treated with Tiaopi Anchang recipe combined with Hufu Tongtou Guasha on the basis of the treatment,and the course of treatment was four weeks.The efficacy of the two groups was compared,and the changes of GCSI,PAC-SYM,gastric emptying time and gastric emptying rate,electrogastric electrogram parameters,blood glucose and quality of life were recorded before and after treatment in the two groups.Adverse reactions and recurrence rates were recorded in both groups.Results:The total effective rate of observation group was higher than control group(P<0.05).Compared with before treatment,the GCSI score,PAC-SYM score,percentage of gastric electrical rhythm disorder,blood glucose index,PAC-QOL were significantly reduced after treatment in the two groups,gastric emptying rate,preprandial and postprandial mean frequency of contraction(MFC)and amplitude contraction(AC)were increased(all P<0.05),and the above indexes in observation group were better than control group(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The recurrence rate in observation group was lower than control group(P<0.05).Conclusion:Tiaopi Anchang recipe combined with Hufu Tongbian Guasha can improve the clinical efficacy of constipation caused by DGP in the elderly,which can not only alleviate the symptoms of patients,improve gastric emptying ability and gastrointestinal motility,but also regulate blood glucose,improve the quality of life of constipation patients,reduce the recurrence rate,and has good safety.
