银屑病患者脂肪因子、BMI与代谢综合征发病的相关性

摘　　要：目的:评价患者体质指数(BMI)及血清IL-23、vaspin、RBP-4水平与银屑病伴发代谢综合征(MS)的相关性。方法:120例寻常型银屑病患者分为银屑病合并MS组和单纯银屑病组,比较两组间BMI、IL-23、vaspin、RBP-4水平的差异,分析BMI及以上脂肪因子对银屑病患者发生MS风险的诊断价值。结果:与单纯银屑病组相比,银屑病合并MS组患者BMI、vaspin、RBP-4水平均升高,差异有统计学意义(P<0.05)。银屑病患者中,BMI每增加1 kg/m^(2),MS发病风险增加1.681倍(OR:1.681,95%CI:1.330~2.125);vaspin每下降1 ng/mL,MS患病风险增加2.833倍(OR:0.353,95%CI:0.160~0.780);RBP-4每增加1μg/L,银屑病患者的MS发病风险增加1.203倍(OR:1.203,95%CI:1.011~1.431)。两组IL-23水平比较无明显差异(P>0.05)。结论:BMI和血清vaspin、RBP-4是银屑病发生MS的危险因素。临床上可根据上述指标预测银屑病患者伴发MS的风险并早期采取针对性干预措施,降低银屑病患者的MS发病率。Objective:To investigate the correlation of body mass index(BMI),serum levels of IL-23,vaspin and RBP-4 with metabolic syndrome(MS)in patients with psoriasis.Methods:120 patients with psoriasis vulgaris were divided into psoriasis combined with MS group and psoriasis alone group.The differences of BMI,IL-23,vaspin and RBP-4 between the two groups were compared,and the diagnostic value of BMI and above adipokines for the risk of MS were analyzed.Results:Compared with psoriasis group,the BMI,serum levels of vaspin and RBP-4 in psoriasis group combined with MS were increased,with statistical significance(P<0.05).In psoriasis patients,for every 1 kg/m^(2) increase of BMI,the risk of MS increased by 1.681 times(OR 1.681,95%CI 1.330-2.125).For every 1 ng/mL decrease of vaspin,the risk of MS increased by 2.833 times(OR 0.353,95%CI 0.160-0.780).For every 1μg/L increase of RBP-4,the risk of MS in patients with psoriasis increased by 1.203 times(OR 1.203,95%CI 1.011-1.431).There was no significant difference of IL-23 level between the two groups(P>0.05).Conclusion:BMI and serum vaspin and RBP-4 are the risk factors for MS in patients with psoriasis.According to the above indicators,the risk of MS in patients with psoriasis can be predicted and early targeted intervention measures can be taken to reduce the incidence of MS in patients with psoriasis.
