重组贻贝粘蛋白联合他克莫司软膏与抗组胺药治疗面部糖皮质激素依赖性 皮炎的疗效及对皮肤屏障指标的影响

摘　　要：目的:研究重组贻贝粘蛋白联合他克莫司软膏、抗组胺药治疗面部糖皮质激素依赖性皮炎(简称面部皮炎)及对皮肤屏障指标的影响。方法:选取2020年4月-2022年12月笔者医院门诊面部皮炎患者131例,采用随机数字表法分为对照组(n=65)与研究组(n=66)。两组均对面部皮炎进行常规处理,对照组给予他克莫司与氯雷他定治疗,研究组在对照组基础上联合重组贻贝粘蛋白治疗,持续治疗4周后,比较两组皮肤屏障指标(表皮皮脂含量、角质层含水量,经表皮水分丢失量)、自觉症状与客观体征积分、疗效、不良反应。结果:治疗后两组表皮皮脂含量、角质层含水量上升,经表皮水分丢失量下降,且研究组表皮皮脂含量、角质层含水量高于对照组,经表皮水分丢失量低于对照组(P<0.05)。治疗4周后两组自觉症状与客观体征积分下降,且研究组积分低于对照组(P<0.05)。研究组总有效率93.94%高于对照组81.54%(P<0.05)。两组总不良反应发生率比较,差异无统计学意义(P>0.05)。结论:重组贻贝粘蛋白联合他克莫司软膏、抗组胺药能有效改善面部皮炎患者临床症状,促进恢复皮肤屏障功能,且无明显严重不良反应发生。Objective Study on the efficacy of recombinant mussel mucin combined with tacrolimus and antihistamines in the treatment of facial hormone dependent dermatitis(referred to as facial dermatitis)and its impact on skin barrier indicators.Methods One hundred and thirty-one patients with facial dermatitis referred to dermatology outpatient clinic in the author's hospital from April 2020 to December 2022 were enrolled,and assigned into two groups according the random number table methods.All patients received routine interventions,based on this,control group(n=65)was given tacrolimus plus antihistamine therapy,while study group(n=66)received recombinant mussel adhesive protein combined with tacrolimus and antihistamine.Treatment in both groups lasted for 4 weeks.Then the outcome criteria including skin barrier function(casual sebum content,stratum corneum hydration,transepidermal water loss),scores of objective clinical signs and subjective symptoms,clinical efficacy,and adverse reactions were compared between two groups.Results After treatment,an increase in casual sebum content and stratum corneum hydration along with a decrease in transepidermal water loss were observed in both groups,and study group had higher casual sebum content and stratum corneum hydration,and lower transepidermal water loss than those in control group(P<0.05).After discontinuation of the drug,the scores of objective clinical signs and subjective symptoms decreased in both groups,and study group scored lower than control group(P<0.05).The overall efficacy rate was 93.94%in study group,which was higher than 81.54%in control group(P<0.05).The adverse reaction rate yielded no statistical difference between two groups(P>0.05).Conclusion Application of recombinant mussel adhesive protein combined with tacrolimus ointment and antihistamine in the treatment of facial dermatitis can effectively improve the clinical symptoms,promote the restoration of skin barrier function and cause no serious adverse effects.
