吻合器痔上黏膜环切术治疗混合痔嵌顿的疗效和安全性观察

摘　　要：目的:评价吻合器痔上黏膜环切术治疗混合痔嵌顿的疗效性和安全性。方法:选取2020年9月至2021年9月,本院收治的88例混合痔嵌顿患者,采用抽签法随机分为观察组和对照组,每组各44例。观察组采用吻合器痔上黏膜环切术治疗,对照组采用传统外剥内扎术治疗,分析比较2组临床疗效、手术时间、创口愈合时间及并发症等情况。结果:观察组手术时间为(14.5±1.1)min,低于对照组的(44.5±2.3)min(P<0.05);创口愈合时间为(2.1±1.1)d,短于对照组的(8.7±2.3)d(P<0.05);出血量为(12.4±1.1)mL,少于对照组的(44.5±2.3)mL(P<0.05);并发症发生率为2.2%,低于对照组的20.4%(P<0.05);临床总有效率为97.7%,高于对照组的77.2%(P<0.05)。结论:采用吻合器痔上黏膜环切术治疗混合痔嵌顿临床疗效确切,能减少并发症的发生,安全性好,值得临床推广。Objective To evaluate the efficacy and safety of procedure for prolapse and hemorrhoids(PPH)in the treatment of mixed hemorrhoidal incarceration.Methods Total 88 patients with mixed hemorrhoidal incarceration admitted to our hospital from September 2020 to September 2021 were random-ly divided into observation group and control group by drawing lots,with 44 cases in each group.The ob-servation group was treated with PPH,while the control group was treated with Milligan-Morgan hemor-rhoidectomy(MMH).The clinical efficacy,operative time,wound healing time and complications were an-alyzed and compared between the two groups.Results The operation time was(14.5±1.1)min in the observation group,which was shorter than(44.5±2.3)min in the control group(P<0.05).Wound healing time was(2.1±1.1)d in the observation group,which was shorter than(8.7±2.3)d in the con-trol group(P<0.05).The amount of blood loss was(12.4±1.1)mL in the observation group,which was less than(44.5±2.3)mL in the control group(P<0.05).The complication rate was 2.2%in the observation group,which was lower than 20.4%in the control group(P<0.05).The total effective rate was 97.7%in the observation group,which was higher than 77.2%in the control group(P<0.05).Conclusion The treatment of mixed hemorrhoid incarceration with PPH has definite clinical effect,and can reduce the occurrence of complications,which is safe and worthy of clinical promotion and application.
