温针灸联合平乐膏药外敷对膝关节骨性关节炎患者膝关节功能、炎症因子水平的影响

摘　　要：目的探讨温针灸联合平乐膏药外敷在膝关节骨性关节炎患者关节功能中的应用效果。方法采用随机数字表法将2020年1月至2022年12月河南省洛阳正骨医院(河南省骨科医院)收治的106例膝关节骨性关节炎患者分为对照组和观察组,各53例。对照组接受平乐膏药外敷,观察组则于对照组的基础上接受温针灸,两组均连续进行治疗4周。比较两组治疗4周后临床疗效,治疗前、治疗4周后中医证候积分、膝关节运动功能Lysholm量表(LKSS)、西安大略和麦克马斯特大学骨关节炎指数、视觉模拟评分(VAS)评分、炎症因子水平和研究期间安全性。结果治疗4周后,观察组的临床总有效率高于对照组。与治疗前比较,治疗4周后两组双腿无力酸软、膝关节疼痛、活动受限、关节肿大各项评分、西安大略和麦克马斯特大学骨关节炎指数、VAS评分、血清白细胞介素-17(IL-17)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)水平均降低,且观察组低于对照组,LKSS评分则均升高,且观察组高于对照组(P<0.05)。两组在观察期内的不良反应发生率差异无统计学意义(P>0.05)。结论温针灸联合平乐膏药外敷可有效提高膝关节骨性关节炎患者的临床治疗效果,改善其临床症状以及膝关节功能,减轻患者的疼痛以及病情,降低患者机体内炎症因子水平,且具有一定的安全性。Objective To explore the application effect of warm acupuncture combined with Pingle plaster on joint function of patients with knee osteoarthritis.Methods A total of 106 patients with knee osteoarthritis admitted in Luoyang Orthopedic-Traumatological Hospital of Henan Province(Henan Provincial Orthopedic Hospital)from January 2020 to December 2022 were divide into the control group and the observation group by random number table method,with 53 cases in each group.The control group applied Pingle plaster externally,while the observation group applied warm acupuncture on the basis of the control group.Both groups were treated for 4 weeks continuously.The clinical efficacy after 4 weeks of treatment,score of traditional Chinese medicine syndrome,Lysholm knee scoring scale(LKSS),Western Ontario and McMaster University osteoarthritis index,visual analogue scale(VAS),level of inflammatory factor before and after 4 weeks of treatment,and the safety during the study period observed were compared between the two groups.Results After 4 weeks of treatment,the total clinical effective rate in the observation group was higher than that in the control group.Compared with before treatment,after 4 weeks of treatment,the scores of weakness and weakness of legs,knee pain,limited activity and joint swelling,Western Ontario and McMaster University osteoarthritis index,VAS,levels of serum interleukin-17(IL-17),hypersensitive C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)in the two groups all decreased,and the levels in the observation group were lower than those in the control group,and LKSS all increased,and the observation group was higher than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups during the observation period(P>0.05).Conclusion Warming acupuncture and moxibustion combined with pingle plaster external application can effectively improve the clinical treatment effect of patients with knee osteoarthritis,improve their clinical symp
