集束化护理干预联合PDCA循环在老年人工髋关节置换术后静脉血栓栓塞症管理中的应用

摘　　要：目的探讨集束化护理干预联合PDCA循环在老年人工髋关节置换术后静脉血栓栓塞(VTE)管理中的应用效果。方法选取中国人民解放军联勤保障部队第九八八医院骨科老年人工髋关节置换术后患者,2022年6—12月收治的患者接受常规护理,为对照组,2023年1—6月收治患者在常规护理的基础上接受集束化护理干预联合PDCA循环护理,为观察组,每组60例患者。对比两组患者治疗期间自护能力、手术后1个月内VTE发生率、股静脉峰值流速和平均流速、患者满意度。结果护理前,两组患者的自护能力评分差异无统计学意义(P>0.05),护理后,观察组患者的自护能力评分、满意度均高于对照组(P<0.05);两组患者在手术前和手术后1 d的股静脉峰值流速和平均流速差异无统计学意义(P>0.05),在手术后7 d时差异有统计学意义(P<0.05);术后1个月内VTE发生率低于对照组(P<0.05)。结论对髋关节置换术后患者实施集束化护理联合PDCA循环,能提升患者的自护能力和满意度,有效降低术后VTE的发生率。Objective To investigate the application of cluster nursing intervention combined with PDCA cycle in the management of venous thromboembolism(VTE)after orthopedic total hip arthroplasty in elderly patients.Methods The elderly patients after hip arthroplasty were selected from the 988th Hospital of the PLA Joint Logistic Support Force.Patients admitted from June to December 2022 received routine care as the control group,and patients admitted from January to June 2023 received cluster nursing intervention combined with PDCA cycle nursing on the basis of routine care as the observation group,with 60 patients in each group.The self-care ability,incidence of VTE within 1 month after surgery,peak velocity and mean velocity of femoral vein,and patient satisfaction during treatment were compared between the two groups.Results Before nursing,there was no difference in the self-care ability scores of patients in both groups(P>0.05).After nursing,the self-care ability scores and satisfaction of patients in the observation group were higher than those in the control group(P<0.05).The peak velocity and mean velocity of femoral vein were comparable between the two groups before surgery and on the first day after surgery(P>0.05),but there were differences on the 7th day after surgery(P<0.05).The incidence of VTE within 1 month after surgery was lower in the observation group than that in the control group(P<0.05).Conclusion Implementing cluster nursing intervention combined with PDCA cycle management for patients after total hip arthroplasty can enhance patients’self-care ability and satisfaction,and reduces the incidence of postoperative VTE.
