摘 要:目的探究并分析振幅整合脑电图联合头颅磁共振成像(MRI)对33例新生儿化脓性脑膜炎患儿预后的预测价值。方法选取2019—2022年温州市中心医院治疗的33例确诊新生儿化脓性脑膜炎的足月新生儿为研究对象,采用Glasgow结局量表评估患儿预后情况,根据预后情况分为预后良好组23例和预后不良组10例。所有患儿均进行振幅整合脑电图联合头颅MRI检查,受试者特征曲线(ROC)分析振幅整合脑电图联合头颅MRI对新生儿化脓性脑膜炎预后的预测价值。结果与预后良好组小儿神经发育筛查量表评估(97.56±3.26)分相比,预后不良组患儿神经发育检查量表(77.45±2.16)分较低,差异有统计学意义(P<0.05)。与预后良好组脑脊液总蛋白含量(1.22±0.14)g/L、白细胞水平(2.14±0.18)×10^(9)/L相比,预后不良组脑脊液总蛋白含量(1.92±0.22)g/L、白细胞水平(3.45±0.44)×10^(9)/L均较高,差异有统计学意义(P<0.05)。与预后良好组振幅整合脑电图21.74%、头颅MRI 26.09%相比,预后不良组患儿的振幅整合脑电图检查出异常比例振幅整合脑电图70.00%、头颅MRI 60.00%较高,差异有统计学意义(P<0.05)。与振幅整合脑电图参数、头颅MRI参数单项预测价值相比,二者联合预测价值较高,差异有统计学意义(P<0.05)。结论振幅整合脑电图联合头颅MRI对新生儿化脓性脑膜炎预后具有较好的预测价值,临床上可观察其异常,积极采取有效治疗措施,改善患儿预后。Objective To explore and analyze the predictive value of amplitude-integrated electroencephalogram(EEG)combined with cranial magnetic resonance imaging(MRI)for 33 cases of neonatal purulent meningitis.Methods A total of 33 full-term neonates diagnosed as neonatal purulent meningitis treated in Wenzhou Central Hospital from 2019 to 2022 were selected,and Glasgow outcome scale was used to evaluate the prognosis.According to the prognosis,33 neonates were divided into good prognosis group(23 cases)and poor prognosis group(10 cases):All the children underwent amplitude integrated EEG combined with cranial MRI examination,receiver operator characteristics(ROC)curve was used to analyze the predictive value of amplitude-integrated EEG combined with cranial MRI for neonatal purulent meningitis.Results Compared with good prognosis group(97.56±3.26),the score of neurodevelopmental scale(77.45±2.16)in poor prognosis group was lower(P<0.05).Compared with good prognosis group[cerebrospinal fluid(CSF)total protein content(1.22±0.14)g/L,leukocyte level(2.14±0.18)×10^(9)/L],poor prognosis group had higher CSF total protein content[(1.92±0.22)g/L]and leukocyte level[(3.45±0.44)×10^(9)/L],with statistical difference(P<0.05).Compared with good prognosis group(amplitude integrated EEG:21.74%;cranial MRI:26.09%),the proportions of children with abnormal amplitude integrated EEG(70.00%)and cranial MRI(60.00%)in poor prognosis group were higher,with statistical difference(P<0.05).Compared with single prediction value of amplitude integrated EEG parameters and cranial MRI parameters,the combined prediction value of the two methods was higher,with statistical difference(P<0.05).Conclusion Amplitude integrated EEG combined with cranial MRI has a good predictive value for prognosis of neonatal purulent meningitis,which can be observed clinically,and effective treatment measures can be actively taken to improve the prognosis of children.
