摘　　要：肝纤维化是存在于大多数慢性乙型肝炎(CHB)疾病过程中的病理变化,其本质是一种可逆的肝组织损伤过度修复反应,肝纤维化的持续存在可导致肝硬化、肝癌的发生风险明显增加。延缓、逆转肝纤维化进程,可改善CHB患者预后。大量研究表明中医药在逆转肝纤维化过程中发挥了重要作用。王灿晖教授认为其基本病机为正虚邪盛,邪毒久稽,肝络受损,气滞血瘀。邪正斗争贯穿于CHB的始终,肝纤维化的进展是机体正气和湿热疫毒、湿浊、血瘀等长期相互作用的结果。主要治则为扶正祛邪。王教授通过健脾养肝补肾、清热利湿解毒、活血祛瘀理气之扶正祛邪法来治疗CHB肝纤维化,治疗过程中临证变通,慎用苦寒药、活血行气药、滋腻之品,多获良效。Hepatic fibrosis is a pathological change that exists in most chronic hepatitis B diseases(CHB).Its essence is a reversible liver tissue damage and excessive repair reaction.The continuous existence of hepatic fibrosis can significantly increase the risk of liver cirrhosis and liver cancer.Delaying and reversing the progression of hepatic fibrosis can improve the prognosis of CHB patients.Numerous studies have shown that traditional Chinese medicine plays an important role in reversing hepatic fibrosis.Professor Wang Canhui believes that the basic pathogenesis of this disease is the deficiency of righteousness and the excess of evil,prolonged accumulation of evil toxins,damage to the liver meridians,and stagnation of qi and blood stasis.The struggle between evil and righteousness runs through the entire process of CHB,and the progression of liver fibrosis is the result of the long-term interaction between the body's healthy qi,damp-heat and epidemic toxins,dampness and turbidity,and blood stasis.The main treatment is to reinforce Healthy Qi and Dispelling Pathogen.Professor Wang treats CHB and hepatic fibrosis through the methods of strengthening the spleen,nourishing the liver,tonifying the kidney,clearing heat,removing dampness,detoxifying,promoting blood circulation,removing blood stasis,and regulating qi.During the treatment process,he adapts to different symptoms and uses bitter cold medicine,blood activating and qi promoting medicine,and nourishing greasy substances with caution,achieving many good effects.
